EMPLOYEE INSURANCE - RENEWAL OF HEALTH AND LIFE <br />INSURANCE CONTRACTS - BLUE CROSS BLUE SHIELD, <br />BRADMAN/UniPSYCH, HIGHMARK LIFE INSURANCE, AND SAFECO <br />(LINCOLN NATIONAL HEALTH & CASUALTY INSURANCE <br />COMPANY) <br />The Board reviewed a Memorandum of September 11, 2002: <br />TO: James E. Chandler <br />County Administrator <br />FROM: James Camicella <br />Human Resource erector <br />DATE September 11, 2002 <br />SUBJECT: Insurance Contract Renewals <br />Staff requests consideration of the following information at the September 17, 2002, regular <br />meeting of the Board of County Commissioners. <br />Background <br />The County's insurance benefits are provided by Blue Cross Blue Shield of Florida (health <br />benefit plan claims administration, utilization review services, and prescnption benefits plan), <br />Bradman/UniPsych (Employee Assistance Program and mental health benefits), Highmark Life <br />Insurance (basic and optional life insurance), and Safeco (excess/stop loss insurance). <br />Recommendation <br />Staff recommends the Board execute the attached renewal contracts with the above-mentioned <br />insurance companies for fiscal year 10/01/02 - 09/30/03. <br />ON MOTION by Commissioner Adams, SECONDED BY <br />Commissioner Ginn, the Board unanimously approved the <br />renewal contracts and authorized the Chairman to execute same <br />with the above insurance companies for FY 2002-2003, as <br />recommended in the memorandum. <br />EMPLOYER APPLICATIONS FOR 6 EMPLOYEE GROUPS AND <br />CONTRACTS ARE ON FILE <br />IN THE OFFICE OF THE CLERK TO THE BOARD <br />September 17, 2002 <br />75 <br />Use <br />• <br />
