REPORTING: What will you do with this information to show that change has <br /> occurred ? How will you use or present these results to the consumer, the funder, the <br /> program , and the community? How will you use this information to improve your <br /> program ? <br /> The numbers for the point system will be complied on a weekly basis and entered in to a <br /> database accordingly. From the database we will be able to chart our outcomes to see when : <br /> improvements in behaviors are being made. We will be keeping all the returned feedback sheets <br /> for one year, to reference as needed . Information collected for pre and post parenting classes is <br /> complied and then entered in to a database . We will be able to chart the results and accurately <br /> measure outcomes . The information is shared with the counselors and parents of the child in <br /> camp, with a signed release of information form . It may also be shared with educators and <br /> therapists, to help better serve the child ' s needs . <br /> 12 <br />
