s ti <br /> 5 <br /> EXHIBIT 1 <br /> Performance Contract <br /> Supportive Services Contract Staff at Stark Family Center <br /> Project Description — FL29B 109002 <br /> The project will provide Supportive Services Staff to assist up to 16 families with children per year for <br /> three years and eight additional families per year not living at the Stark Family Center. Families in the <br /> program will receive staff assistance and case management to (a) obtain and maintain permanent <br /> employment, (b) secure permanent affordable housing and (c) achieve a level of economic and social <br /> self-sufficiency that will end homelessness . <br /> Outcomes for Clients <br /> Over the three-year grant period, the program will demonstrate the following outcomes for clients of <br /> the program as a whole : <br /> • 75 % will complete the goals and objective of their Housing and Supportive Services Plan <br /> within twelve months of establishing residence at Family OPTIONS . <br /> • 80% of will increase job readiness and job seeking knowledge <br /> • 80% will establish a realistic job goal <br /> • 80% will obtain gainful employment <br /> • 40% will maintain employment for a minimum of six months <br /> • 85 % will report an increase in self esteem and self confidence in the employment arena <br /> • 90% will obtain permanent affordable housing within one year <br /> • 85 % will sustain permanent affordable housing for more than one year. <br /> Staff Performance <br /> Over the three-year grant period, Case Management Staff will perform the following assessments and <br /> services where applicable for all clients identified as participants in this grant program: <br /> • Housing : Length of homelessness, employment history, current entitlements, previous <br /> homelessness, type of housing needed. <br /> • Life Management Skills : Consumer skills, financial management skills, community re- <br /> integration skills, property and home maintenance skills . <br /> • Medical & Mental Health Status : History, medications, current status, immunizations, <br /> physical disability, mental health status, domestic violence, substance abuse history, etc . <br /> • Parenting & Child Care Skills : Daycare needs , children ' s health needs, children' s <br /> educational needs, parenting skills, conflict management, history of neglect or abuse, etc . <br /> • Motivation & Commitment to Self-Sufficiency : Subjective evaluation, clinical observation, <br /> follow through history, recidivism etc . <br /> • Employment : Work history, work adjustment problems, educational level, job readiness, job <br /> seeking & job keeping skill level, specialized skills, vocational interests, aptitudes and <br /> abilities . <br />
