� - Ik - o3 ti /l G03 PROFESSIONAL CIVIL ENGINEERING SERVICES C) S ` o L 1 AGREEMENT FOR CIVIL ENGINEERING AND BRIDGE DESIGN SERVICES THIS AGREEMENT, entered into this 18th day of March 2003 by and between INDIAN RIVER .COUNTY, a political subdivision of the State of Florida, hereinafter refereed to as the COUNTY, and BRIDGE DESIGN ASSOCIATES , INC. , Vero Beach, Florida, hereinafter referred to as the ENGINEER, WITNESSETH The COUNTY and the ENGINEER, in consideration of the mutual covenants, herein agree with respect to the performance of professional civil engineering, bridge design and related services by the ENGINEER and the payment for those services by the COUNTY as set forth below. The ENGINEER shall provide professional engineering and related services for the COUNTY in all phases of the project described in SECTION III-Scope of Services; serve as the COUNTY ' S professional representative for the project; and shall give professional engineering and land surveying advise to the COUNTY during the performance of the services to be rendered. SECTION I - PROJECT LIMITS AND DESCRIPTION Consultant services are required for the preparation of design surveys, roadway improvement plans, bridge replacement plans and permit applications for the replacement of three bridges and associated roadway improvements. The three bridge locations are 5th Street SW over 43 `d Avenue Ditch (No. 884048), Old Dixie Highway over South Relief Canal (No . 880031 ), and 43`d Avenue over South Relief Canal (No. 880045 ) . The associated roadway and bridge improvements at each location are as follows : 5th Street SW over 43 `d Avenue Ditch — the COUNTY desires to replace the existing two-lane structure on 5d' Street SW with a three-lane structure. In addition, left-turn lanes will be designed on the east approach, south approach and north approach to the 5d' Street SW and 43 `d Avenue Intersection. Realignment will be designed for 5d' Street SW east of 43 `d Avenue to align with 5d' Street SW west of 43 `d Avenue. The approximate total length of all approaches is '/z mile . Any required change in roadway elevation, to accommodate the bridge vertical grade, is anticipated to be accommodated within the above listed roadway improvements. Old Dixie Highway over South Relief Canal — the COUNTY desires to replace the existing two- lane structure on Old Dixie Highway to with a three-lane structure. In addition, a three-lane roadway section will be designed between I 't Street and 2nd Street. The approximate total length
