ADDENDUM #9 TO CONTRACT DATED JANUARY 21, 1997 BETWEEN INDIAN RIVER <br />COUNTY AND MASTELLER & MOLER, INC. FOR CR 512 PAVING & DRAINAGE <br />IMPROVEMENTS — PHASE IV <br />INDIAN RIVER COUNTY PROJECT 49611 <br />COUNTY ROUTE 512 CORRIDOR IMPROVEMENTS <br />ROSELAND ROAD TO 1-95 <br />INDIAN RIVER COUNTY, FL <br />Indian River County staff has diligently attempted to secure right-of-way from various property owners <br />along County Route 512 for construction of "Four -lane Roadway Improvements" between Roseland Road <br />and Interstate -95 based on the original Project design. As a result of difficulties experienced with the <br />right-of-way acquisition process, the County has made a decision to realign portion of the proposed <br />roadway improvements. <br />This Addendum for Phase IV will be defined as follows: <br />Perform update of approximately 1,800 linear feet of the existing Route Survey for County Road <br />512, Phase IV, Indian River County, Florida and revise Contract Plans and Cross Sections in order <br />to correctly reflect existing conditions. <br />Topographic Survey and Plan Revisions include the following. <br />1. Perform cross sections of approximately 1,800 linear feet of the recently repaved and <br />regarded areas in the vicinity of 101x` Avenue within the limits of the Phase IV County <br />Road 512 project. <br />2. Update the existing route survey and Contract Plans to correctly indicate existing <br />conditions. <br />We propose to perform the Scope of Services outlined under Task A for the following lump sum fees: <br />Land Surveying & Plan Revisions <br />$4,825.00 <br />We will complete the above described Scope of Services in a diligent manner after our receipt of written <br />authorization to proceed from Indian River County. <br />1N WITNESS WHEREOF the parties hereto have executed these presenis this 16th day of <br />May , 2006. < <br />MASTELLER & MOLER, INC. <br />1655 27d' Street, Suite 2 <br />By: <br />Earl H.
