2 . Environmental Permits : <br /> The ENGINEER shall prepare stormwater permit applications for the St. Johns <br /> River Water Management District and US Environmental Protection requirements <br /> for submittal by the COUNTY . This will consist of all required evaluation, <br /> design, coordination, and follow-up work necessary to support permit <br /> applications . The COUNTY will review the permit applications as necessary. <br /> The ENGINEER shall assemble and be responsible for the final submittal . <br /> The ENGINEER shall prepare permit sketches for submission by the COUNTY to <br /> Florida Department of Environmental Protection, U. S . Army Corps of Engineers, <br /> and St . Johns River Water Management District for Dredge and Fill activities, if <br /> necessary. The ENGINEER shall submit all permit sketches on 8 " x 11 " sheets . <br /> Sketches shall be neatly scaled, signed and sealed, and reproducible. <br /> 3 . Utilities : <br /> Utility coordination with the various Utility Companies and/or agencies shall be <br /> by the ENGINEER and identified conflicts addressed. The ENGINEER will be <br /> prepared, at additional hourly rates, to survey the project site to locate and <br /> determine depth of underground utilities, in coordination with the Utility <br /> Companies . <br /> 4 . A Stormwater Pollution Prevention Plan (SWPPP) will be developed in <br /> conjunction with this project . The site specific SWPPP is a requirement of both <br /> the Environmental Protection Agency (EPA) National Pollutant Discharge <br /> Elimination System (NPDES) General Permit for Stormwater Discharges from <br /> Construction Activities and the State of Florida Department of Environmental <br /> Protection (DEP) Generic Permit for Stormwater Discharges from Construction <br /> Activities . <br /> F . Signing and Pavement Markin Pg lans <br /> Preparation of the plan layout; key map; quantities (including signing and pavement <br /> marking quantity) ; tabulation of quantities ; and detail of all major signs . All plans are to <br /> be prepared in accordance with the latest design standards and practices (MUTCD) , <br /> FDOT Standard Specifications, Indexes, and shall be accurate, legible, complete in design <br /> and drawn at the same scale as the Roadway Plans, furnished in reproducible form. <br /> G . Signalization Plans <br /> Traffic signal modifications will be designed to accommodate the roadway improvements <br /> to the 27"' Avenue intersection . The signal modifications will be designed to meet current <br /> COUNTY and FDOT mast arm standards . <br /> H . Construction Phase Assistance <br /> Construction phase assistance shall be submitted to the COUNTY under a supplemental <br /> Work Order, if so requested, once the construction documents have been finalized and a <br /> construction duration has been determined. <br /> I . Certification <br /> 1 . The ENGINEER will provide certification to St . Johns River Water Management <br /> District based on As-Built drawings provided by the Contractor ' s licensed <br /> surveyor and information provided by the COUNTY. <br /> 7 <br />
