III. Consulting Engineer Insurance Requirements <br /> A. Workers Compensation as required by the State of Florida. Employer's Liability of <br /> $100,000 each accident, $500,000 disease policy limit, and $100,000 disease each <br /> employee. <br /> B. General Liability: commercial general liability coverage, including contractual <br /> liability and independent contractor, with a minimum combined single limit of <br /> $100,000 per occurrence and personal injury coverage of $50,000. <br /> C. Business Automobile Liability : owned, hired, and non-owned vehicles at a <br /> minimum combined single limit of $100,000 per occurrence and personal injury <br /> coverage of $50,000 for bodily injury and property damage for owned and non- <br /> owned vehicles. <br /> D. Professional Liability Insurance: providing coverage for negligent acts, errors, or <br /> omissions committed by Consultant with a limit of $1,000,000 per claim/ annual <br /> aggregate. This insurance shall extend coverage to loss of interest, earning, profit, <br /> use, and business interruption, cost of replacement power, and other special, <br /> indirect, and consequential damages. <br /> IV. Compensation for Services <br /> The additional expense associated with reconfiguring the wellheads to allow for <br /> sampling and the increased frequency have added to the previously anticipated project <br /> cost. The original work order consisted of 20 hours of labor per month for sampling and <br /> reporting for a total of 480 hours. After beginning the sampling process, it was <br /> determined that all wells could be sampled in a 4-hour period, rather than a full 8-hour <br /> day. Increasing the number of sampling events from 24 to 104 is expected to increase the <br /> number of hours required from 480 to 896. A summary of the hours/budget in the <br /> original proposal, as well as the amended amounts to account for the modified scope of <br /> services is provided in Table 1. The additional cost incurred from reconfiguring the <br /> wellheads was approximately $750 for time and materials. The budgeted other direct <br /> costs did not include provisions for purchasing pressure gauges, sampling taps, fittings, <br /> etc. The total requested increase in the upper limit under this amendment is $46,510. <br /> Compensation is to be paid on a billing rate basis plus other direct costs on an as-needed <br /> basis.
