OC� o `1os� <br /> INDIAN RIVER COUNTY <br /> 5`h STREET SW BRIDGE OVER LATERAL " C " CANAL <br /> 43 " AVENUE BRIDGE OVER SOUTH RELIEF CANAL AND <br /> OLD DIXIE HIGHWAY BRIDGE OVER SOUTH RELIEF CANAL <br /> AMENDMENT NO, 2 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br /> SERVICES AGREEMENT BETWEEN BRIDGE DESIGN ASSOCIATES , INC. AND INDIAN <br /> RIVER COUNTY, FLORIDA. <br /> This is an amendment to the existing Engineering/Land Surveying Services Agreement (AGREEMENT) <br /> dated March 18 , 2003 between Bridge Design Associates, Inc . (ENGINEER) and Indian River County <br /> (COUNTY) . This amendment addresses changes in "Section I" — "Project Limits and Description", <br /> " Section III - Scope of Services " and " Section V - Compensation" . <br /> Amendment Description <br /> This Amendment includes the following : <br /> The ENGINEER will perform the necessary engineering services to combine the glans that have been <br /> prepared for the intersection improvements at the intersection of 43 `d Avenue and 5 Street SW with the <br /> plans for the replacement of the 43 `d Avenue Bridge over the South Relief Canal, and to widen 43 `d <br /> Avenue to a three lane section from north of Oslo Road to Vineyard Lane SW. <br /> "SECTION I — PROJECT LIMITS AND DESCRIPTION <br /> The south limit of the project is located approximately 757 feet north of the intersection of Oslo Road <br /> and 43 `d Avenue where the existing 3 - lane section starts to transition north to a 2-lane section. The north <br /> limit of the project is at Vineyard Lane SW where the existing 3 -lane section transitions south to a 2-lane <br /> section . The total length of the project on 43 `d Avenue is approximately . 82 miles . The east-west project <br /> limits on 5t" St SW are outlined in the original AGREEMENT. <br /> " SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following : <br /> The plans for the improvement of the intersection of 43 `d Avenue and 5th SW and for the replacement of <br /> the 43 `d Avenue Bridge over the South Relief Canal will be combined into one set of construction plans <br /> suitable for bidding and construction. Additional plan sheets will be prepared covering the new project <br /> limits and detailing the widening and reconstruction of 43 `d Avenue . <br /> A Stormwater Pollution Prevention Plan (SWPPP) will be developed in conjunction with this project. <br /> The site specific SWPPP is a requirement of both the Environmental Protection Agency (EPA) <br /> National Pollutant Discharge Elimination System (NPDES) General Permit for Stormwater Discharges <br /> from Construction Activities and the State of Florida Department of Environmental Protection <br /> (DEP) Generic Permit for Stormwater Discharges from Construction Activities. <br /> A traffic signal will be designed to accommodate the intersection improvements to the 5th Street SW/43rd <br /> Avenue intersection.
