Improvements <br /> (S .R. 60 to 4th Street) <br /> Addendum No. 4 — Professional Services Agreement <br /> IRC Job # 98- 10/CAI Job # 98-23E <br /> Dear Chris : <br /> We are pleased to present this proposed addendum for additional services relative to breaking the <br /> construction plans package into two separate construction packages and preparing construction plan <br /> revisions and obtaining permit modifications resulting from the County' s right-of-way acquisition <br /> of the Beatty properties. All provisions of the original Professional Engineering Services Agreement <br /> (dated November 17, 1998), including Addendum Nos. 1 , 2 and 3, shall remain in full force and <br /> effect, unless otherwise specifically modified herein. A manhour breakdown is attached as Exhibit <br /> "A" for your use. The following describes our proposed scope of services and terms of agreement <br /> between Carter Associates, Inc . (CAI) and Indian River County (COUNTY): <br /> Separation of Construction Documents into Two Separate Packages : <br /> In general, the dans will be divided into two separate construction packages. The north package <br /> will include 66' Avenue improvements from and including 16th Street to SR60 . The south package <br /> will include the remainder of the project. The construction packages will be prepared to be <br /> independent of each other for the purposes of bidding and constructing the improvements <br /> separately. <br /> Fees for Separation of Construction Documents are. . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . .$ 13 ,540. 00 <br /> Design Modifications : <br /> In general, the plans will be modified to incorporate the following new items: <br /> • Add a 12" drainage culvert between the Basile and Beatty properties under 12th Street. <br /> • Replace the existing 12" drainage culvert under 8h Street at the boundary of the Beatty and <br /> Rourke properties. <br /> • Add 22 ' driveway to garage at the south section of the proposed circular drive @ <br /> approximately Sta 39+00 66th Avenue; The driveways are to be constructed of concrete. <br /> • Relocate to the south the entry driveway to the proposed asphalt parking pad located @ <br /> approximately Sta 45+00 66th Avenue. <br /> • Replace existing drainage culvert @ approximately Sta 48+00 66th Avenue. <br /> • Add 12 ' asphalt access drive @ approximately Sta 66+00 66th Avenue.
