Kafer, Jr. , P.E. , County Engine <br /> SUBJECT: Amendment No. 5 to the Professional Services Agreement with Carter <br /> Associates, Inc. for Roadway Improvements, 66th Avenue from 4th Street to <br /> SR 60 — IRC Project No. 9810 <br /> DATE: January 16, 2007 <br /> DESCRIPTION AND CONDITIONS <br /> Indian River County entered into a Professional Services Agreement with Carter Associates, Inc. on <br /> November 17, 1998 and approved Amendment No. 1 through Amendment No . 4 for additional <br /> service. <br /> Additional services are now required in order to accommodate the connection of College Lane at the <br /> alignment of 16th Street. The proposed bridge for College Lane over the I .R.F. W. C .D. Lateral A <br /> Canal will be required to be much higher than the existing bridge, which requires 66th Avenue to be <br /> much higher for proper tie-in. <br /> A summary of the proposed work for this Amendment is as follows : <br /> • Revise 16`h Street and 66th Avenue Plan & Profile sheets, Cross Sections, and Signing <br /> & Pavement Marking Plan sheets. <br /> • Revise drainage structures, drainage piping, and stormwater management report. <br /> • Revise Tabulation of Quantities and Summary of Drainage Structures. <br /> • Update engineer' s cost estimate . <br /> Breakdown summary of design costs for the project are as follows : <br /> Original Contract Price $ 66,000.00 <br /> Amendment No. 1 $ 95 ,000. 00 <br /> Amendment No. 2 $ 175820. 00 <br /> Amendment No . 3 $ 118 ,800 . 00 <br /> Amendment No. 4 $ 30,640.00 <br /> Proposed Amendment No . 5 $ 21 ,460. 00 <br /> NEW CONTRACT PRICE $ 349,720.00 <br /> FAEngineering\Capital Projects\9810-66th Ave, 4th St to SR 6019810 BCC Agn-Amend 5-01 - 16-07-cjk.doe <br />
