Permittee : INDIAN RIVER COUNTY BOARD OF COUNTY COMMISSIONERS <br /> Permit No : 200001872 ( IP - IS ) <br /> Page 9 <br /> 22 . The permittee agrees to provide a planting plan /monitoring plan <br /> for dune restoration and enhancement of beach mouse habitat , for our <br /> review and approval within 90 days of issuance of this permit <br /> instrument . <br /> 23 . The permittee agrees to subject the designated pipeline corridors <br /> to diver - towed videography prior to construction . . Any portions of the <br /> pipeline directly crossing exposed nearshore hardbottom will be fitted <br /> with collars . The permittee agrees to include the results of the <br /> pipeline corridor monitoring with the first annual biological <br /> monitoring report . <br /> 24 . The permittee agrees to comply with the Standard Hopper Dredge <br /> Conditions , Jacksonville District Regulatory Division dated October <br /> 15 , 2002 ( attachment # 9 ) . <br /> 25 . The permittee agrees to subject certain specific areas of the <br /> project location to a limited survey . The areas selected for cultural <br /> surveys would be limited to heavy equipment access corridors , and <br /> booster pump locations as identified on the final construction. plans <br /> for the project . If significant cultural resources are identified in <br /> these selected areas the access points and/or booster pump locations <br /> will be relocated . In addition the permittee agrees to establishing <br /> 200 - foot buffer zones around two magnetic anomalies within the South <br /> Borrow area identified in a cultural resources survey Dr . Robert Baer <br /> entitled Submerged Cultural Resources Remote Sensing Survey Pursuant <br /> to Beach Restoration of Three Borrow Areas Offshore of Indian River <br /> County , Florida , CRAT No . 2000 - 10229 . The resultant survey report will <br /> be forwarded to: the Florida Department of State , Division of <br /> Historical Resources , Attn : Janet Snyder Matthews , PH . D . , . State <br /> Historic Preservation Officer , 500 S . Bronough Street , Tallahassee , <br /> Florida 32399 - 0250 , prior to the commencement of activities authorized <br /> in this permit instrument . <br /> 26 . within 60 days of the authorized work and completion of the <br /> artificial hardbottom , the attached Self - Certification Statement of <br /> Compliance must be completed and submitted to the U .
