BRIDGE REPLACEMENTS AND WIDENING OF 66TH AVENUE <br /> FROM SOUTH OF SR 60 TO 59TH STREET <br /> PROFESSIONAL CIVIL ENGINEERING, GEOTECHNICAL ENGINEERING <br /> AND LAND SURVEYING SERVICES <br /> AMENDMENT NO . 5 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br /> SERVICES AGREEMENT BETWEEN ARCADIS AND INDIAN RIVER COUNTY, <br /> FLORIDA. <br /> This is an amendment to the existing Engineering/Land Surveying Services Agreement <br /> (AGREEMENT) dated September 6 , 2005 between ARCADIS (ENGINEER) and Indian River <br /> County (COUNTY) . This amendment addresses changes in " Section I — Project Limits and <br /> Description" , " Section III - Scope of Services " , and " Section V - Compensation" . <br /> Amendment Description <br /> This Amendment includes the following : <br /> " SECTION I - PROJECT LIMITS AND DESCRIPTION" is being modified to incorporate <br /> the following : <br /> The County is planning to construct the segment of 66th Avenue from 16th Street to SR 60 . <br /> Consultant services are required for modifications to the plans to provide for a separate set of <br /> construction plans for the segment of 66th Avenue from south of SR 60 to approximately 500 feet <br /> north of SR 60 . The plans will be modified to provide for a temporary connection to the existing <br /> 66t' Avenue roadway north of SR 60 . In addition, a plan for the temporary diversion of traffic <br /> from the existing undivided roadway north of SR 60 to the divided roadway on the south side of <br /> SR 60 , <br /> The St. Johns River Water Control District has requested that a Cultural Resource Assessment be <br /> prepared for the project . Panamerican Consultants will prepare the Cultural Resource <br /> Assessment . The limits of the Cultural Resource Assessment will be from South of SR 60 to 59th <br /> Street . Deliverables will include copies of the Cultural Resource Assessment sent to ARCADIS , <br /> Indian River County, and to DHR for review. An electronic copy of the report will be provided . <br /> The plans for 66th Avenue from SR 60 to 26th Street will be revised to reduce the width of the <br /> median in order to reduce the need for right of way and temporary construction easement <br /> acquisition from Vista Plantation . <br /> A separate set of construction plans will be prepared detailing the improvement of 33rd Street <br /> from west of 66th Avenue to 66th Avenue. ARCADIS has prepared plans for the ultimate <br /> intersection of 66th Avenue and 33rd Street. Right of way acquisition has not been finalized at this <br /> time.
