li r <br /> �1 <br /> ,�o c fir_ 3c6 <br /> IMPROVEMENT OF THE INTERSECTION OF SR 60 & 43RD AVENUE <br /> PROFESSIONAL CIVIL ENGINEERING & STRUCTURAL ENGINEERING <br /> SERVICES <br /> AMENDMENT NO , 5 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br /> SERVICES AGREEMENT BETWEEN ARCADIS G & M , Inc . AND INDIAN RIVER <br /> COUNTY, FLORIDA . <br /> This is an amendment to the existing Engineering Services Agreement (AGREEMENT) dated <br /> September 6 , 2005 between ARCADIS G& M , Inc . ( ENGINEER) and Indian River County <br /> ( COUNTY) . This amendment addresses changes in " Section I — Project Limits and Description" , <br /> " Section III - Scope of Services " , and " Section V - Compensation " . <br /> Amendment Description <br /> This Amendment includes the following : <br /> " SECTION I - PROJECT LIMITS AND DESCRIPTION" is being modified to incorporate <br /> the following : <br /> Consultant services are required for the preparation of road design plans ; bridge plans ; permit <br /> applications and utility relocations associated with the construction of SR 60 / 43rd Avenue <br /> Intersection Improvements . <br /> A conceptual site plan and a boundary and location survey is required for use in the preparation <br /> of cure plans for the Paradise Mobile site . The conceptual site plan will depict the existing <br /> conditions at the site , and will detail the proposed site improvements after the acquisition of right <br /> of way for the intersection improvements . The conceptual site plan will detail the proposed <br /> driveways , parking areas , propane tank, signage , and other related site features . <br /> A conceptual commercial area improvement plan will be prepared illustrating the conditions <br /> along 43rd Avenue south of SR 60 following the acquisition of the right of way and the widening <br /> of the roadway . This plan will include the following sites : <br /> • Paradise Mobile <br /> • Szechuan Palace <br /> • Mermaid Car Wash <br /> • Cumberland Farms <br /> • Pinewoods II Animal Hospital <br /> In addition , the plan will show the existing buildings , driveways and front parking areas for the <br /> businesses located on the east side of 43rd Avenue between 19`h Street and SR 60 .
