TO OSLO ROAD (9T11STREETS .W.) ` <br /> WORK ORDER NO. 4 TO THE PROFESSIONAL CIVIL ENGINEERING SERVICES <br /> AGREEMENT BETWEEN ARCADIS U . S . INC . AND INDIAN RIVER COUNTY, FLORIDA. <br /> This Work Order No . 4, entered into this 3RD day of Mflrch , 2009 , by and between <br /> INDIAN RIVER COUNTY , a political subdivision of the State of Florida, hereinafter referred to as the COUNTY , <br />and <br /> ARCADIS U . S . , INC . , Vero Beach , Florida, hereinafter referred to as the ENGINEER, modifies the original Agreement as <br /> follows : <br /> " SECTION III — SCOPE OF SERVICES " <br /> Consultant services are required for Environmental Assessment and Regulatory Permitting in <br /> support of the preparation of road design plans and permit applications for the construction of the <br /> widening of 43rd Avenue from 25 `" Street S . W . to Oslo Road (9`" Street S . W . ) . <br /> B . Scope of Services : <br /> The preparation of environmental assessment and regulatory permitting documents in response to the US Army <br /> Corps of Engineers request for additional information. The environmental assessment and regulatory <br /> permitting documents will consist of documentation relative to wetland jurisdictional services and endangered <br /> and threatened/species of special concern including: <br /> • Field determination of approximate wetland lines <br /> Approximate wetland boundary depiction <br /> Preparation of a wetland jurisdictional summary memorandum <br /> • Endangered & threatened /species of special concern surveys <br /> • Agency coordination <br /> If required by the USACE, a permitting submittal package will be prepared. The submittal package will consist <br /> of.• <br /> • UMAM <br /> • Mitigation alternatives analysis <br /> • Environmental Resource Permit (ERP) Application <br /> • Coordination with Army Corps of Engineers <br /> If required by the USA CE, a mitigation plan will be prepared. <br /> The flagging and surveying of wetland boundaries in the field, if required by the regulatory agencies, is not <br /> included in this work authorization. This work will be included in a subsequent work authorization if requested <br /> by the County. <br /> " SECTION IV - TIME FOR COMPLETION" is being modified to incorporate the following : <br /> The time for completion of the Environmental Assessment and Regulatory Permitting for the <br /> project shall be two (2) months from the date of execution of this Work Order No . 4 . <br /> Page l of 3 <br /> C : ADocuments and Settings\hdeibel\Local Settings\Temporary Internet Files\Content.
