EXHIBIT 1 <br /> EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Abatement, Demolition and Removal Contract <br /> I. OBJECTIVE : <br /> The objective of this Agreement is to retain a General Contractor to provide abatement, <br /> demolition and/or removal services necessary along the Florida Department of <br /> Transportation, Turnpike Enterprise' s right-of-way . The General Contractor is responsible <br /> for obtaining an Florida Asbestos Consultant to perform the asbestos work. <br /> The Department shall request contractor services on an as-needed basis. <br /> The Department shall authorize such services on a parcel(s) basis, based on need and <br /> availability of budget. No minimum amount of work is guaranteed under this contract. This is <br /> a non-exclusive contract. Other contractors may be utilized. <br /> II. CONTRACTOR'S REQUIREMENTS: <br /> Contractor, at the time of presenting a proposal, must have the capabilities of maintaining a <br /> minimum of three (3) satellite offices in addition to their main office. <br /> Contractor is required to have a minimum of five (5) years experience with demolition <br /> contracts. The demolition contracts awarded to Contractor should be a minimum of $ 100,000 <br /> per contract. <br /> Contractor must hold a Florida Class "A" General Contractor license. <br /> Contractor must hold a Florida State Certified Asbestos Contractor' s License. <br /> M. CONTRACTOR' S RESPONSIBILITY: <br /> The Contractor must carry and keep in force during the period of this contract a general <br /> liability insurance policy or policies with a company authorized to do business in the state of <br /> Florida, affording public liability insurance with combined bodily injury limits of at least <br /> $ 1 .000 ,000.00 per person and $500.000 .00 each occurrence, and property damage insurance <br /> of at lease $ 1 ,000,000.00 each occurrence, for the services to be rendered in accordance with <br /> this contract. <br /> The Contractor' s Asbestos Sub-Consultant must have and maintain during the period of <br /> this contract, a professional liability insurance policy or policies or an irrevocable letter of <br /> credit established pursuant to Chapter 675 , Florida Statutes, and Section 337 . 106, Florida <br /> Statutes, with a company authorized to do business in the state of Florida, affording <br /> professional liability coverage for the professional services to be rendered in accordance with <br /> this contract in the amount of at least $ 1 .000.000.00. The Contractor shall maintain <br /> professional liability coverage for a minimum of three years after completion of the services <br /> rendered under this contract. <br /> The Contractor shall provide the Department with a Performance Bond in the amount <br /> of $450,000.00/full amount of the proposal). The Performance Bond shall be provided by <br /> a surety company authorized to do business in the state of Florida. The Performance <br /> Bond shall be executed and furnished to the Department prior to contract execution and <br /> no later than ten (10) days after the ending date of the period for posting the intended <br /> award decision, unless the Department extends the time period in writing.
