q <br /> AMENDMENT NO. 3 TO THE PROFESSIONAL <br /> ENGINEERING SERVICES AGREEMENT <br /> 66TH AVENUE PAVING AND DRAINAGE IMPROVEMENTS <br /> BETWEEN SR 60 AND 4TH STREET <br /> IRC PROJECT NO , 9810 <br /> THIS is an amendment to the existing AGREEMENT dated NOVEMBER 17, 1998 , including <br /> AMENDMENT NUMBER 1 AND AMENDMENT NUMBER 2, between CARTER ASSOCIATES , INC . <br /> (ENGINEER) and INDIAN RIVER COUNTY (COUNTY) . The proposed scope of work generally includes <br /> final design of roadway and drainage improvements, permitting, and construction phase services . The re- <br /> design will be based on the new alignment prepared under Amendment No . 2 of the existing agreement . All <br /> provisions of the original Professional Engineering Services Agreement including Amendment Nos . 1 and 2 <br /> shall remain in full force and effect, unless otherwise specifically modified. This amendment addresses <br /> changes in "FINAL DESIGN AND PERMITTING", "CAI CONSTRUCTION PHASE SERVICES <br /> (ROADWAY AND DRAINAGE)", "JENKINS & CHARLAND CONSTRUCTION PHASE SERVICES <br /> (BRIDGE)", "COMPENSATION" , and "TE\/M OF COMPLETION ' of the AGREEMENT . <br /> SCOPE OF SERVICES <br /> See attached letter from Carter Associates , Inc . dated April 2 , 2004, which specifies changes in the <br /> " SCOPE OF SERVICES " and "COMPENSATION", the manhour breakdown attachment EXHIBIT A, and <br /> the attached current Rate Schedule from Carter Associates, Inc . <br /> COMPENSATION <br /> This section of the AGREEMENT entitled " SECTION V - COMPENSATION" shall be revised in <br /> response to the Scope modifications set out herein. The charges associated with this AMENDMENT NO . 3 <br /> are as follows : <br /> Fees for Final Design and Permitting : $93 , 000 . 00 (Lump Sum) <br /> Construction Phase Services (Roadway and Drainage) : N. T. E . $20, 000 . 00 (Time & Materials) * <br /> *Fees used for this work shall be per attached Rate Schedule dated February 2004 .
