I, <br />. <br />Board of County Commissioners <br />Administration - Building A (South) <br />1801 27th Street <br />Vero Beach, Florida 32960 <br />Telephone: (772) 567-8000 FAX: 772-778-9391 <br />Project: <br />San Sebastian Specific Purpose Record Survey <br />IRC Project No. UPC 3014 <br />WORK ORDER NO. 6, (Utilities/Surveying) <br />FOR <br />PROFESSIONAL SERVICES AGREEMENT <br />ANNUAL SURVEYING and MAPPING/GIS SERVICES <br />WITH <br />Carter Associates, Inc. <br />In accordance with Contract No. 0725 <br />This Work Order No. 6 is in accordance with the existing AGREEMENT dated November 6`h <br />2007 between Carter Associates, Inc. (SURVEYOR) and Indian River County (COUNTY). This <br />Work Order No. 6 amends the agreement as follows: <br />SECTION I — PROJECT LIMITS <br />This Work Order No. 6 is for the SURVEYOR to perform a "Specific Purpose Record Survey' of <br />the Platted roadways within the subdivisions of San Sebastian Springs and San Sebastian <br />Lakes as further identified in the attached "Exhibit A". <br />SECTION II - SCOPE OF SERVICES <br />As agreed upon between Carter Associates, Inc. and Indian River County, the SURVEYOR <br />shall provide Professional Land Surveying services to complete all tasks as outlined in this Work <br />Order No. 6 described within the attached "Exhibit A": <br />SECTION III —TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT <br />1. Project shall <br />be completed <br />within thirty <br />(30) business days from Notice to Proceed. <br />2. Deliverables <br />to include five <br />(5) signed <br />and sealed hard copies and electronic copies <br />in AutoCAD <br />2007 and PDF <br />formats. <br />SECTION IV — COMPENSATION <br />The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as <br />identified in Sections I, II, and III of this Work Order No. 6, a lump sum fee of $24,400.00 <br />Attachment 1 <br />1 OF 4 <br />
