PROPOSAL <br /> EXHIBIT "A" <br /> WORK ORDER NO . 6 <br /> FOR PROFESSIONAL SERVICES AGREEMENT ANNUAL SURVEYING and MAPPING/GIS SERVICES <br /> CONTRACT N0. 0725, 2007.2009 <br /> SCOPE : <br /> GENERAL DESCRIPTION <br /> Project consists of approximately 13, 000 linear feet of road rights-of-way located within five <br /> subdivisions : <br /> 1 . San Sebastian Springs ( PB 11 , Pg 58 & 92 ) <br /> 2 . San Sebastian Springs Unit II ( PB 12 , Pg 91 ) <br /> 3 . San Sebastian Springs Unit III ( PB 13, Pg 53 ) <br /> 4. San Sebastian Lakes ( PB 13, Pg 54) <br /> 5. San Sebastian Springs Unit IV ( PB 13 , Pg 55) <br /> A Specific Purpose Record Survey will be conducted for the purposes of identifying and <br /> mapping existing improvements within the platted rights-of-way and contiguously platted utility <br /> easements . These improvements shall be all above ground appurtenances including , but not <br /> limited to pavements , curbs, walks, manholes, valve covers , inlets, culverts , driveways , mail <br /> boxes and cable utility boxes . An AutoCadd ( version 7) base map will be generated including <br /> the platted right-of-ways , easements and lot lines with the addresses of all houses for the <br /> purposes of the County's engineering design and contractor as-builts . <br /> FIELD SURVEY <br /> • Field locate right-of-way centerline monumentation necessary to determine boundaries of <br /> subject roadways . Survey control will be performed by GPS , based upon NAD 83/99 horizontal <br /> datum . <br /> • Field locate all above ground appurtenances within the platted rights-of-ways and <br /> contiguously platted utility easements . <br /> • Establish elevations on found centerline control based upon NAVD 88 vertical datum to <br /> best accuracies available utilizing GPS procedures. <br /> • Cross section rights of way at 100 foot intervals. <br /> • Field verify all house addresses . <br /> FINAL MAPPING <br /> • Compute record plat rights-of-ways and contiguous easements, orient to field control . <br /> • Identify all field located improvements by symbol and/or text description . <br /> • Establish centerline stationing and geometry. <br /> • Publish State Plane Coordinates and elevations on all located centerline control . <br /> • Identify lot numbers and house addresses. <br /> • Provide certified map(s ) and AutoCadd (version 7) file on disc. <br /> TOTAL LUMP SUM $ 24,400 . 00 <br /> 3OF4 <br />
