(See individual funder requirements for <br /> inclusion of collaborative agreement letters.) <br /> Collaborative Ageng Resources provided to the program <br /> Indian River County Health IRCHD provides free personnel services, supervision, <br /> Department payroll services, storage space, cleaning, office space, <br /> All resources provided in-kind conference rooms, fax and phone service and access to <br /> without an administrative fee. office equipment for copying, printing and postage. <br /> Indian River County Healthy Start 1RHSC provides program development, training <br /> Coalition opportunities, integration and consistent communication <br /> among programs and the community, grant writing and <br /> All resources provided in-kind development, fundraising; PR, QA/QI, budgeting and <br /> without an administrative fee. fiscal accountability, marketing, presentations, and <br /> representation of TLC in the community. <br /> Indian River Medical Center Provides access to mothers with strict adherence to <br /> confidentiality guidelines; storage space for TLC <br /> All resources provided in-kind paperwork, manuals and references, positive promotion <br /> without an administrative fee. of TLC by hospital staff, access to labor and delivery <br /> staff, meeting rooms, and communication between <br /> medical providers and TLC staff. <br /> IRC Healthy Start Care Coordination Care coordinators raise awareness of TLC through <br /> Program contact with mothers of babies bom outside of IRC and <br /> coordinate services between TLC and Healthy Start <br /> depending on the family' s risk factors. For families at <br /> risk, Healthy Start will provide case management for up <br /> to three years if needed; serving families who are not <br /> capable of managing the risk themselves (i .e. teen <br /> moms, homeless, multiple births, medical needs, <br /> developmental delays, domestic violence, drug or <br /> alcohol abuse, postpartum depression). Healthy Start can <br /> provide mental health and nutrition counseling. <br /> IRC Healthy Families Program Family Support Workers begin during the prenatal <br /> period to link parents to TLC services and Wee Wisdom <br /> newsletter. They will provide parenting guidance <br /> through structured home visits; teach parents the Great <br /> Growing Kids curriculum, promote positive parent <br /> interaction and enhance parenting skills and the child' s <br /> development. <br /> MomCare Program The Maternity Care Advisor is aware of high risk <br /> mothers delivering outside of IRMC and encourages <br /> families to call TLC and is able to share contact <br /> information with client permission. <br /> 10 <br />
