(See individual funder requirements for <br /> inclusion of collaborative a reement letters.) <br /> Collaborative Agency Resources provided to the program <br /> Indian River County Health • Provides coordination of HS/HF prenatal screen. <br /> Department • Communication on potential HF client' s status. <br /> Services are provided in-kind. • Medical/nursing based services for HF client on <br /> an "as-needed" basis. <br /> • WIC and needed health care services at CHD <br /> clinics . <br /> Indian River Memorial Hospital • The hospital works with the Healthy Start <br /> Services are provided in-kind. Screening Liaison to facilitate the Universal <br /> Screen, provides space and a computer. <br /> IRC Library — Born to Read • Provides two books and a book-bag to families <br /> Program with a newborn who participate in HF IRC . HF <br /> Services are provided in-kind. IRC emphasizes the importance of reading to <br /> their infant to enhance brain development. <br /> Partners in Women' s Health • Primary site for Healthy Families referrals. <br /> Services are provided in-kind. Partner' s employees provide HF IRC orientation, <br /> education, and processing of the Universal <br /> Healthy Start Screen. <br /> Indian River County Healthy Start • Overall program development, integration and <br /> Coalition communication within all IRCHSC programs. <br /> • Office Space at no cost to HF IRC. <br /> The Coalition provides in-kind • PR and marketing of HF IRC Program <br /> support to the HF IRC program • QA/QI oversight. <br /> • Accounting, fiscal reporting and audit <br /> preparation. <br /> • HF IRC representation at public events. <br /> • Development and presentation of HF IRC grants . <br /> Storage Solutions • Provides confidential document storage <br /> Services are provided in-kind. <br /> Martha's Circle • Volunteers for family graduation and provides <br /> Community Church holiday gifts for children and cash assistance for <br /> participants. <br /> 10 <br />
