List the major action steps , activities, or cycles of events that will occur within the <br /> program year. New programs should include any start-up planning that may occur <br /> outside the funding year. In completing the timetable, review information detailed in <br /> prior sections . <br /> Month/Period Activities <br /> June 2004 Begin advertising and contacting referral sources . Create a list of <br /> potential group members orienting them to the group process . <br /> July 2004 Develop registration form that includes all demographics . Compile <br /> research and develop curriculum of each group therapy program . <br /> Develop pre and post evaluations . Screen potential group members <br /> have them complete intake form <br /> August 2004 Contact groups members inform them of the first meeting <br /> September 2004 Begin first group session . Take attendance, complete pre-test and <br /> begin the group process <br /> December 2004 Children ' s group will follow same format as adolescent group except <br /> their cycle will be five weeks with four cycles per semester ( 18 weeks) <br /> Adolescent Groups will end. Groups members will complete post-test <br /> evaluation form <br /> Develop the second 18 week group curriculum tailoring it to the needs <br /> of the identified group population, Compile goals of group members <br /> and determine outcomes <br /> January 2005 - Give parents demographic form to complete and patients pretest <br /> June 2005 evaluation form, compile goals and chart data <br /> Second 18 week groups begin, initial group curriculum based on the <br /> group member ' s need identified by intake/registration form . <br /> Compile post test data evaluation information, compile outcomes for <br /> each group <br /> Wrap up group data for grant information (employee paycheck, <br /> June 2005 cancelled checks , finance department. <br /> 14 <br />
