Organization: Exchange Club CASTLE Program Name: Safe Families <br /> Funder: Children' s Services Advisory Committee RFP# 2006061 <br /> G . TIMETABLE (Section G not to exceed one page) <br /> 1 . List the major action steps, activities, or cycles of events that will occur within the <br /> program year. New programs should include any start-up planning that may occur <br /> outside the funding year. In completing the timetable, review information detailed in <br /> prior sections. <br /> Month/Period Activities <br /> October 1 , 2006 Safe Families is a continuing program and will be fully staffed and in <br /> full operation at the start of the contract year. Regarding the program <br /> operation: <br /> 1 . Referred families are contacted within 48 hours. <br /> 2 . Referred families are assessed within 14 days . <br /> 3 . A family plan is developed within 30 days . <br /> 4. Weekly visits take place for up to one year. <br /> 5 . Monthly and quarterly progress reports track client progress . <br /> 6. Post testing and protective/risk factor assessments take place near <br /> the end of services. <br /> 7 . Follow-up is done for three months after case closure. <br /> October 1 , 2006 8 . Parent education services (groups) are offered to families who do <br /> not need the intensity of home visitation services, are on a waiting <br /> list, or need additional follow-up in a supervised setting to remain <br /> abuse free. <br /> 13 <br />
