ATTACHMENT VII <br /> Primary Care <br /> " Primary Care" as conceptualized for the county health departments and for the use of categorical <br /> Primary Care funds (revenue object code 015011 ) is defined as : <br /> " Health care services for the prevention or treatment of acute or chronic medical conditions or minor <br /> injuries of individuals which is provided in a clinic setting and may include family planning and <br /> maternity care . " <br /> Indicate below the county health department programs that will be supported at least in part with <br /> categorical Primary Care funds this contract year : <br /> . x Comprehensive Child Health (229/29 ) <br /> x Comprehensive Adult Health (237/37 ) <br /> x Family Planning (223/23 ) <br /> Maternal Health/IPO (225/25 ) <br /> Laboratory (242/42 ) <br /> Pharmacy (241 /93 ) <br /> Other Medical Treatment Program (please identify) <br /> Describe the target population to be served with categorical Primary Care funds . <br /> The target population served with categorical Primary Care funds are the residents of Indian River <br /> County , who fall at or below 200 % of Federal Poverty . <br /> Does the health department intend to contract with other providers for the delivery of primary health <br /> care services using categorical (015011 ) Primary Care funds ?
