Attachment D <br /> Reporting Requirements <br /> The following reports must be completed and submitted to the Department in the time frame <br /> indicated. Failure to timely file these reports constitutes an event of default, as defined in Paragraph ( 10) <br /> of this Agreement. <br /> 1 . The Contractual Obligation and MBE Report must be submitted to the Department by <br /> April 15 and October 15 annually. The form must reflect all contractual activity for the period. If <br /> no activity has taken place during the reporting period, the form must indicate "no activity". <br /> 2. The Request for Funds ( RFF) Form must be submitted to the Department electronically <br /> at a minimum of once per quarter. If no activity has taken place during the reporting period for <br /> which funds will be requested, the form must indicate, " no funds are required" . A breakout of <br /> expenditures by service area must be submitted that supports expenditures reflected on the <br /> RFF. These service area breakouts must be received and reviewed prior to approving the RFF <br /> for processing. <br /> 3. The Projection of Contract Payments and Report on Accomplishments To Date Form <br /> must be submitted to the Department (4) times a year: May 1 , August 1 , November 1 and <br /> February 1 . In Section I Funding Projection , indicate the amount of funds that will be required for <br /> the applicable reporting period. If no funds will be required , a zero should be reflected in the <br /> applicable reporting period . In Section II Accomplishments, all accomplishments must be <br /> reported from implementation to date of report. <br /> 4 . The Project Generated Income Report, if applicable, must be submitted to the <br /> Department on a quarterly basis on the form provided by the Department. The form must reflect <br /> all earned or expended income and show the available balance as of the end of the reporting <br /> period . <br /> 30 <br />
