2 • S • o `�; <br />Page I of 2 <br />Board of County Commissioners <br />1810 25th Street <br />Vero Beach, Florida 32960 <br />Telephone: (772) 567-8000 FAX: (772)-778-9391 <br />Field and Laboratory Quality Control (QC) <br />for <br />CR512, PHASE IV <br />IRC Project No, 9611 <br />WORK ORDER NO. 2 (Engineering Division) <br />FOR <br />FOR PROFESSIONAL GEOTECHNICAL SERVICES AGREEMENT <br />FOR ANNUAL GEOTECHNICAL SERVICES CONTRACT NO. 0534 <br />WITH <br />DUNKELBERGER ENGINEERING & TESTING, INC. <br />All services performed under this contract shall be supervised and certified by a licensed <br />Professional Engineer (PE) registered with the State of Florida and staff employed directly with <br />your firm. <br />Scope of Services <br />All work shall be performed on a unit price basis in accordance with the attached Scope of <br />Services dated January 25, 2008 and shall comply with Standards enumerated in the <br />RENEWAL OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT IRC PROJECT NO. <br />0534 WITH DUNKELBERGER EGINEERING & TESTING INC. Effective Date: October 1, <br />2007. <br />Minimum Specification Requirements as follows: <br />Roadway Construction <br />In-place moisture density testing of embankment fill, pipe backfill, stabilized sub grade, <br />base course, and curb pads. <br />Laboratory testing of soils and aggregates including Proctors, Limerock Bearing Ratio <br />(LBR), soil classification, carbonate content (base), and other such specified QC tests. <br />Preparation and issuance of engineering reports presenting the test results and their <br />compliance with the project specifications. <br />All work shall also comply with the current Florida Minimum Technical Standards. <br />COMPENSATION. <br />The COUNTY agrees to pay, and the Geotechnical Professional agrees to accept, a fee for <br />services rendered according to the "Scope of Services" identified in the attached EXHIBIT "l <br />Fee Schedule and Man Hours. <br />C ll•�dnni o <br />'! :J S<Iwi , ),I lc )� II �QFii[I_� IQmp..u, lm.Ill, l IIu.l)] h> Ill)I l;: i_ %'Ild,I V.41 , U.irl-Invr_,i 1 '113at. <br />
