PAS 3 tic) <br /> 5F� <br /> OP <br /> 0 Ak C <br /> ma` <br /> CY <br /> EMERGENCY MEDICAL SERVICES <br /> CLASS "A <br /> CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY <br /> WHEREAS , the Town of Indian River Shores, Department of Public Safety; has requested authorization to provide BLS/ALS Emergency <br /> Medical Services to the citizens of the Town of Indian River Shores; and <br /> WHEREAS , a need has been demonstrated to provide these essential services to the citizens of the Town of Indian River Shores; and <br /> WHEREAS , the above named service affirms that it will maintain compliance with the requirements of the Emergency Medical Act, Chapter <br /> 401 , Florida Statutes; Florida Administrative Code, Chapter 64E-2 ; and Chapter 304, Indian River County Code of Laws and Ordinances. <br /> THEREFORE, the Board of County Commissioners of Indian River County hereby issues a Class A " Certificate of Public Convenience <br /> and Necessity to the Town of Indian River Shores, Department of Public Safety; to provide BLS/ALS Emergency Medical Services with <br /> limitations as prescribed on this certificate. <br /> Effective Date: April 13, 2006 <br /> Date of Expiration: April 13, 2008 <br /> RI�JER C (unless certificate is sooner revoked or suspended) <br /> Limitations: Primary response service area <br /> limited to the Town of Indian River Shores. <br /> Arthur R.
