MEASURABLE OUTCOMES (Entire Section D not to exceed two pages) <br /> OUTCOMES ACTIVITIES <br /> Add all the elements or the Measurable Outcome(s) Add the tasks to accomplish the Outcome(s) <br /> 1 . Ensure that visits occurring at Valued Visits 1 .
