Organization: Exchange Club CASTLE Program: Safe Families <br /> Funder: Children' s Services Advisory Committee RFP: #5054 <br /> 3. REPORTING : What will you do with this information to show that change has <br /> occurred ? How will you use or present these results to the consumer, the funder, the <br /> program, and the community? How will you use this information to improve your <br /> program? <br /> Information collected is used to measure the progress of enrolled families , and to help determine <br /> the length and content of the intervention . Families are aware of their progress on the AAPI, <br /> risk/protective factor checklist, and family plan goals . <br /> Client satisfaction surveys are analyzed through the CASTLE ' s CQI process , in the Service <br /> Delivery committee . Recommendations for program improvements come from this committee . <br /> Results from collected information are reported to funders on a regular basis through monthly, <br /> quarterly or semi-annual reports . Staff, Board members , and other stakeholders are made aware <br /> of results through the CQI process , and feedback at all -team and Board meetings . The <br /> community is made aware of results through an annual report . <br /> G. TIMETABLE (section G not to exceed one page) <br /> Safe Families is a continuing program and will be fully staffed and in full operation at the start of <br /> the contract year. Regarding the program operation : <br /> 1 . Referred families are contacted within 48 hours . <br /> 2 . Referred families are assessed within 14 days . <br /> 3 . A family plan is developed within 30 days . <br /> 4 . Weekly visits take place for up to one year. <br /> 2 . Monthly and quarterly progress reports track client progress . <br /> 3 . Post testing and protective/risk factor assessments take place near the end of services . <br /> 4 . Follow-up is done within one year of closure . <br /> Regarding the hiring of an additional counselor: <br /> 1 . Advertise, interview and screen candidates mid August through mid September. <br /> 2 . Offer the position to one candidate in mid September. <br /> 3 . Hire the new counselor on October 1 , 2003 . <br /> 12 <br />
