This is a great opportunity for your agency <br /> to better serve your community and region. <br /> Enclosed is a Personnel Qualification Inventory (PQI) form that will help us identify your <br /> training needs . We plan to conduct the training locally for your region. Please complete and <br /> return the PQI form no later than May 28 , 2004 to Thomas Quinn at the below address, or by fax <br /> to 352 732- 1433 , or you can request an electronic PQI form from Tom at quinnt@dfs.state. fl.us <br /> Also enclosed is a sub-grant agreement and an equipment cache list. As part of the sub-grant <br /> agreement, the equipment cache includes all equipment listed with the requirement that your <br /> organization provide medical equipment, building marking supplies and nails (listed on page 3 of <br /> Type IV Required Response Equipment Cache) . Additional optional equipment that is desirable <br /> to better perform the mission, but is not provided by this Agreement may be found in the <br /> document "FASAR Resource Typing Policy 2004" (Florida Association of Search and Rescue) <br /> on their webpage http ://www.ffca.ores/fasar_index .htm.medical tools . <br /> Please review, sign and return the sub-grant agreement and PQI no later than May 28, 2004 <br /> Again, we are pleased that your organization is preparing to provide additional response <br /> capabilities to your area.
