Proposal Landfill <br />Petroleum Resources & Development, Inc. <br />2460 N Courtenay Pkwy, Suite 11 I <br />Merritt Island, Florida 32953 <br />(321)986-6026 <br />Fax (321) 986-6036 <br />lL <br />Quote # 0861-07 <br />We are pleased to offer the following proposal for your consideration. <br />Secure all construction permits for tasks under this scope. <br />Form a slab area 42' x 12'x 12" thick and set rebar at the new location. <br />Pour concrete and finish. <br />Concrete must set for 7 days prior to the tank sitting on it. <br />Unbolt the existing tank, rig the tank for loading onto a truck with a crane. <br />Deliver the tank to the new location, re -rig and offload onto the new slab <br />Go back to old location & break up the slab & remove the bumper post. <br />Backfill the slab area to grade. <br />Go back to the new location and mount the new dispenser & card reader. <br />Install new bumper post's around the tank slab. <br />NOTES: <br />Prior to moving the tank needs to be drained & cleaned and the cleaning is included in the price below. <br />Bumper post's to be painted yellow. <br />Tank assumed empty, (additional cost for vac truck to empty contents, if any). <br />PR&D to supply a high speed dispenser, card reader system, card encoder, modem & 100 cards. <br />County to bring the electric to the slab. <br />This price will remain firm for thirty days from the proposal date <br />We appreciate the opportunity of offering this proposal and <br />trust we may be favored with your most valued business <br />ny Name <br />Signature Date <br />Title <br />Page 1 of 1 <br />Petroleum Resources & Development, Inc <br />
