To complete <br /> reading mentor activities for 6 months or more background screening, interview and <br /> will be promoted to the next grade level by assessment. To obtain client referrals and <br /> June, 2006 . Baseline : 2005 -2006 IRC School parent permission . To interview and pre-test <br /> Board promotion/retention records for enrolled clients . To develop a case plan for each client <br /> boys and girls . documenting needs, goals and action plans to <br /> be reviewed on a quarterly basis . To match <br /> mentors and clients . To provide 1 to 1 <br /> mentoring sessions, one hour weekly in two , <br /> 30 minute intervals to 75 children. To utilize <br /> the social development strategy in emphasizing <br /> learning skills, healthy beliefs, clear standards <br /> and bonding. To review daily mentoring log to <br /> verify weekly visits with the mentor. To <br /> provide an initial 2 hour training block <br /> followed by quarterly training opportunities for <br /> volunteers . To complete match supervision <br /> calls regarding frequency and quality of the <br /> match relationships . To document progress of <br /> match in case notes . To collect all report card <br /> information through IRC Schools . To evaluate <br /> progress throughout the year and document <br /> promotion status . To document grades, <br /> conduct, attendance and promotion status . To <br /> conduct anonymous satisfaction surveys ; <br /> results reported to and discussed with staff and <br /> outside agencies . To close cases where <br /> students have achieved their goals and service <br /> is no longer necessary. To follow-up over a 9 <br /> month period with terminated clients to <br /> determine the long-term impacts of mentoring. <br /> 2 . 80% of the youth who have participated in To administer pre and post testing utilizing <br /> reading mentoring activities for 6 months or DIBLES (Dynamic Indicators of Basic Early <br /> more will improve reading performance in one Literacy Skills).
