Boys & Girls Club of Indian River County, Inc. — Sebastian Branch <br /> Grant Application to the Children's Services Advisory Committee <br /> • School Achievement will be measured by collecting report card data and providing a <br /> comparison of grades from quarter to quarter. A baseline will be established the 1St <br /> quarter of each year and compared to results at the end of the year. <br /> • Parent Surveys * will be distributed to each parent in May to evaluate services being <br /> ` delivered. <br /> • Member Surveys * will be distributed to each member twice per year to determine the <br /> impact the Club has had on them individually. <br /> * Copies of the surveys are attached to this grant application . <br /> 1 . REPORTING : What will you do with this information to show that change has <br /> occurred ? How will you use or present these results to the consumer, the funder, the <br /> program, and the community ? How will you use this information to improve your <br /> program? <br /> All data collected will be compiled and presented to all funders and contributors in order to <br /> present our agencies positive impact on the community. We will use the information gathered as <br /> a result of our measures to continuously update and improve our program to ensure it meets the <br /> needs of our members . <br /> 12 <br />
