List your program ' s collaborative partners and the resources that they are providing to <br /> the program beyond referrals and support. (See individual funder requirements for <br /> inclusion of collaborative agreement letters. <br /> Collaborative Agency Resources provided to the program <br /> CCCR Contracting Therapists : All CCCR therapists discount their hourly rate <br /> Linda Asher, Ph.D . approximately 29% . Some provide evening and <br /> Madeleine Laplante, M . A. , LMHC weekend hours . Some provide service at the child ' s <br /> Therese Cirner, M .A . NCC center. All complete the appropriate Mental Health <br /> Brent A. Jeremy, L . C . S . W. Provider Forms or Psychological Support Classroom <br /> Dr. Judith Siegler, Family Therapist Report Forms . <br /> Dr. Robert Brugnoli , Psychologist <br /> Community Church Partner' s The Partner ' s program can transport CCCR families that <br /> Program they are working with to and from therapy <br /> appointments . The mentor will go with the parent to an <br /> initial appointment, if so desired. The Partner' s Program <br /> will pay for any medication that may be prescribed, if <br /> the parent cannot afford it. In addition, they will <br /> provide babysitting so a parent may go to a therapy <br /> appointment. <br /> Indian River Public Schools and Conduct diagnostic screening on CCCR children who <br /> Florida Diagnostic & Learning have .learning and behavioral concerns . The data is then <br /> Resources System (FDLRS) used by the CCCR therapist to design a treatment plan. <br /> Indian River Early Learning Coalition CCCR manages psychological enhancement dollars for <br /> the Early Learning Coalition. CCCR coordinates <br /> referrals and therapist placements . The CCCR Mental <br /> Health provider Form , the Initial Classroom Observation <br /> Report, and the Psychological Services Survey are used <br /> by the Coalition . The collaboration provides direct <br /> psychological services for subsidized families and <br /> increased revenue for CCCR therapists willing to reduce <br /> rates for both programs . <br /> Application for 2005 -2006 service period 10 <br />
