5 • �� <br /> CD <br /> EMERGENCY MEDICAL SERVICES <br /> CLASS " A " <br /> CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY <br /> WHEREAS , the Town of Indian River Shores , Department of Public Safety ; -has requested authorization to provide BLS/ALS Emergency <br /> Medical Services to the citizens of the Town of Indian River Shores ; and <br /> WHEREAS , a need has been demonstrated to provide these essential services to the citizens of the Town of Indian River Shores ; <br /> and <br /> WHEREAS , the above named service affirms that it will maintain compliance with the requirements of the Emergency Medical Act, Chapter 401 , <br /> Florida Statutes ; Florida Administrative Code, Chapter 64E-2 ; and Chapter 304 , Indian River County Code of Laws and Ordinances . <br /> THEREFORE, the Board of County Commissioners of Indian River County hereby issues a Class " A " Certificate of Public Convenience and <br /> Necessity to the Town of Indian River Shores , Department of Public Safety ; to provide BLS/ALS Emergency Medical Services with limitations <br /> as prescribed on this certificate . <br /> Effective Date : April 13 , 2004 �V CO Date of Expiration : April 13 , 2006 <br /> (unless certificate is sooner revoked or suspended) <br /> Limitations : <br /> PrimM response service area limited <br /> to the Town of Indian River Shores . <br /> Caroline D .
