MEASURABLE OUTCOMES (Description of Intent) <br /> OUTCOMES ACTIVITIES <br /> Add all the elements or the Measurable Outcome(s) Add the tasks to accomplish the Outcome(s) <br /> Note : These outcomes and activities are based <br /> on the standard HF Florida and national HF <br /> program guidelines and requirements . <br /> 1 . Ninety (90) percent of target children will be 1 . FSW ' s will address immunization schedules <br /> fully immunized by age two (2) . with family and record usage . <br /> 2 . Ninety (90) percent of target children will be 2 . The Well Baby checkups will be monitored, <br /> up-to-date with Well-Baby Checks . with the infant ' s medical home established. <br /> Families will be encouraged to seek and utilize a <br /> pediatrician and/or clinic for ongoing medical <br /> care for infant and child. <br /> 3 . Ninety (90) percent of the children in 3 . FSW ' s will address proper parenting skills <br /> families who participant in HF-IRC for six with the family, as well as Shaken Baby <br /> months or longer will have no findings of some Syndrome Education, partner interaction and <br /> indications or verified child maltreatment while anger management. <br /> receiving Healthy Families services . <br /> 4.
