ORIGINAL <br /> INDIAN RIVER COUNTY <br /> Oslo Road Bridge Replacement <br /> And Roadway Widening <br /> AMENDMENT NO . 2 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br /> SERVICES AGREEMENT BETWEEN KIMLEY-HORN AND ASSOCIATES , INC . AND <br /> INDIAN RIVER COUNTY, FLORIDA. <br /> This is an amendment to the existing Engineering/Land Surveying Services Agreement (AGREEMENT) <br /> dated March 24th, 1998 between Kimley-Horn and Associates, Inc . (ENGINEER) and Indian River <br /> County (COUNTY) . This amendment addresses changes in " Section I - Project Limits and Description" , <br /> Section II - County Obligations, " Section III - Scope of Services " , " Section V - Compensation" <br />and <br /> " Section XVI — Duration of Agreement" of the AGREEMENT . <br /> Amendment Description <br /> This Amendment includes the following : <br /> " SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following : <br /> The ENGINEER will revise the Oslo Road and 43rd Avenue roadway plans such that Oslo Road is <br /> constructed as a five lane roadway section (ultimate roadway section) for approximately 4W feet <br /> west of 43rd Avenue at which point it would transition to the existing two lane roadway section' 55 <br /> Based upon the completed construction documents, this revision would effect the following plan <br /> sheets : <br /> Plan and Profile — 2 Sheets <br /> Cross Section — 3 Sheets <br /> Intersection Plateau — 1 Sheet <br /> Signing and Marking — 1 Sheet <br /> Utility Plan — I Sheet <br /> Construction Phasing — 2 Sheets <br /> Utility Design <br /> The Engineer will develop construction documents to detail the relocation of the existing 20" <br /> water main and 10 " force main required to facilitate the proposed bridge replacement over the <br /> Lateral ` C ' Canal . <br /> Utility Permitting <br /> The Engineer will prepare and submit documentation to request a FDEP domestic wastewater and <br /> public drinking water permit. The consultant will coordinate with FDEP in regards to obtaining <br /> these permits .
