FOR
PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Masteller , Moler , Reed & Taylor , Inc .
In accordance with Contract No . 0725 2007- 2009
This Work Order No . 12 is in accordance with the existing AGREEMENT dated November 6th
2007 between Masteller , Moler, Reed & Taylor , Inc . (SURVEYOR ) and Indian River County
( COUNTY) . This Work Order No . 12 amends the agreement as follows :
SECTION I — PROJECT LIMITS
This Work Order No . 12 is for the SURVEYOR to perform surveying services in connection with
the 66th Ave Roadway Improvements Project, Right of Way acquisitions , all lying in Indian River
County , Florida .
SECTION 11 - SCOPE OF SERVICES
As agreed upon between Masteller , Moler, Reed & Taylor , Inc . and Indian River County , the
SURVEYOR shall provide Professional Land Surveying services to complete all tasks as
outlined in the attached Proposal , Scope of Services and man- hours EXHIBIT A.

SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT
1 . Project shall be completed as follows :
a . FINAL project completion shall be within 10 calendar days of Notice to Proceed .
2 . Deliverables -The SURVEYOR shall provide the COUNTY :
a . FINAL five ( 5) paper "hardcopy" 24" x 36" signed and sealed survey .
b . AutoCad drawing file and PDF and DWF formats on CD .
c . Work Product and digital versions are to prepared and submitted so that the
COUNTY or other consultants can readily use it for the design and analysis of
the area , as defined . It shall contain all information necessary for third -party
surveyor to independently recreate and/or utilize the survey work . It is
acknowledged all final products become property of COUNTY and will be
available for use by the public at large .
d . The SURVEYOR ' S work product shall meet or exceed the minimum standards
as defined by Sections II , III and IV or the COUNTY will not approve the
SURVEYOR ' S request for payment.
SECTION IV — COMPENSATION
The COUNTY agrees to pay , and the SURVEYOR agrees to accept, for the above described
services rendered as identified in Sections I , II , and III of this Work Order No . 12 for a total lump
sum fee of $3 , 215 . 00 .
All and/or any additional services not described hereon shall be pre - approved by the
COUNTY . Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year ( s ) of November 6th , 2007
through November 5th , 2009 .
All invoicing shall include project identification , Work Order No . (WO 12) , Contract Number
0725 , IRC project No . 0545 , itemized man - hours and materials expended to complete the scope
of services . Payments shall be in accordance with the original Professional Surveying and
Mapping/GIS Services Agreement , contract No : 0725 with the COUNTY and as stated in
Section II , III and IV hereon .

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREE , „rNT shall remain in full force and effect, and are incorporated herein .
IN WITNESS W-HEREOF the parties hereto have executed these presents this 22nd day
of Sept . 2009 .

MMRT , Inc . is pleased to provide you with this proposal for performance of Boundary Survey for approximately 9 . 5 acre
property located in Indian River County and lying in Section 17 , Township 032 South , Range 039 East and identified as
Tax Parcel 32391700001013000003 . 0 .
1 . Perform boundary survey per legal description as provided . Boundary survey shall be prepared in accordance with the
Minimum Technical Standards for surveying as adopted by the State of Florida .
2 . Locate and identify all improvements on subject property , including but not limited to , buildings , driveways , fences
etc .
3 . Review Title chain package as provided . Show all applicable easements on the survey sketch . No search of the
Public Records will be made by this firm .
4 . Review and verify existing description of right of way Parcel 168 to be acquired .
5 . Depict all existing and proposed rights of way on the survey drawing . Drawing shall also include description of parent
parcel , acquisition parcel and residue parcel as applicable .
6 . Upon completion , certify as requested , provide five copies and a CD containing the CAD file and PDF of the
survey
sketch .
Based upon the man hour estimate attached as Exhibit "A" we propose to perform the above described scope of services
for a lump sum fee of $ 3 , 215 . 00 . We estimate the time of completion to be ten ( 10 ) days from the notice
to proceed .
Upon your verbal authorization for this survey , we order research materials and set up your job file . In the event of
a
cancellation and in order to cover our costs , you will be billed for all work/expenses incurred up to the date of
cancellation ,
including costs to close out your file .
