ATTACHMENT 1 to WORK ORDER NO, M2
LABORATORY SUPPORT SERVICES AND REPORTING FOR THE MAIN
RELIEF CANAL POLLUTION CONTROL SYSTEM
SCOPE OF WORK
The COUNTY has requested that the CONSULTANT provide professional engineering
services for the preparation of reporting requirements as set forth in the Quality Assurance
Project Plan (QAPP) for the COUNTY's Main Relief Canal Pollution Control System
Project. This reporting is required by a Florida Department of Environmental Protection
(FDEP) 319 Grant Contract (DEP Contract No. G0182, dated April 11, 2006). The
following tasks provide the necessary cross-section of activities associated with the
requirements of the QAPP and the FDEP 319 Grant Contract pertaining to the one-year
monitoring period after construction is complete.
Task No. 1 — KICKOFF MEETING - conduct kickoff meeting (telephone conference call)
with COUNTY to discuss report format and assemble pertinent information concerning the
project as it relates to the required reporting requirements.
Task No. 2 — LABORATORY COORDINATION — CONSULTANT will retain the services
of an analytical laboratory that will analyze vegetation and water samples. The vegetation
and water samples will be collected and forwarded to the analytical laboratory by
COUNTY. The subconsultant analytical laboratory will provide a report documenting the
analytical results of each sampling event to CONSULTANT.
Task No. 3 — QUARTERLY REPORTING — CONSULTANT will incorporate laboratory
analytical results into a report, on a quarterly basis, in accordance with the quarterly
reporting requirements of the FDEP 319 Grant Contract, Attachment H. There will be a
total of two quarterly reports submitted for quarters 1 and 3, with the second quarterly
report being incorporated into the Preliminary Project Report as described in Task No. 4
below and the fourth quarterly report being incorporated into the Final Project Report as
described in Task No. 5, below. COUNTY will submit all field notes, water sampling logs
and chain of custody forms to CONSULTANT within two days of each of the 19 sampling
events (12 monthly and 7 storm event sampling events). Water samples will be collected
at all 19 events; however vegetation will only be collected during the 12 monthly events.
Analytical sampling results and field information will be assembled into a report that will be
sent to COUNTY to include in the Quarterly Reporting Requirements of the FDEP 319
Grant Contract. COUNTY will assemble and submit two hard copies of each quarterly
report to the FDEP contract manager upon receipt of the laboratory analytical report from
CONSULTANT.
Task No. 4 — PRELIMINARY PROJECT REPORT — A preliminary report will be prepared
that will assess the effectiveness of the project at the mid -point of the water quality
monitoring program and will reflect a summary of Quarter 1 and Quarter 2 data. The
preliminary project report will address the history and purpose of the project, overview
summary of the project (pre and post -construction), analytical results, and effectiveness

assessment. CONSULTANT will submit

Task No. 5 — FINAL PROJECT REPORT — A final project report will be prepared that will
assess the effectiveness of the project at the end of the water quality monitoring program.
The final project report will address the history and purpose of the project, overview
summary of the project (pre and post -construction), analytical results, and effectiveness
assessment, and will make recommendations for using a similar type project in the future.
CONSULTANT will submit two hard copies and one electronic copy of the draft final
project report to the FDEP contract manager. Upon receipt of comments from FDEP,
CONSULTANT will prepare and submit six hard copies and two electronic copies of the
final project report to COUNTY. COUNTY will include five hard copies and two electronic
copies of the final project report with the fourth quarterly report.

COMPENSATION
The COUNTY agrees to pay and the CONSULTANT agrees to accept, a fee of $35,162.00
for services rendered according to Attachment 1 of this WORK ORDER. Hourly rates shall
be as set forth in the Agreement and a breakdown of the fees is set forth below.

2. PARTIAL PAYMENTS
The COUNTY shall make monthly partial payments to the CONSULTANT for all
authorized work pertaining directly to this project performed during the previous calendar
month. The CONSULTANT shall submit invoices monthly for services performed and
expenses incurred pursuant to this Agreement during the prior month.
The CONSULTANT shall submit duly certified invoices in duplicate to the Director of
the Public Works Department. For lump sum line items, the amount submitted shall be the
prorated amount due for all work performed to date under this phase, determined by
applying the percentage of the work completed as certified by the CONSULTANT, to the
total due for this phase of the work. For time and material line items, the amount
submitted shall be based on the actual hours worked and expenses incurred for the billing
period.
The amount of the partial payment due the CONSULTANT for the work performed
to date under these phases shall be an amount calculated in accordance with the previous
paragraph, less ten percent (10%) of the invoice amount thus determined, which shall be
withheld by the COUNTY as retainage, and less previous payments. Per F.S. 218.74(2),
the COUNTY will pay approved invoices on or before the forty-fifth day after the COUNTY
receives the CONSULTANT's invoice.
Per F.S. 218.74(2), the ten percent (10%) retainage withheld shall be paid in full to
the CONSULTANT by the COUNTY, on or before the forty-fifth day after the date of final
acceptance of the Work by the Public Works Director.

LABORATORY SUPPORT SERVICES AND REPORTING FOR THE MAIN
RELIEF CANAL POLLUTION CONTROL SYSTEM
This Work Order Number M2 is entered into as of this 12th day of February ,
2008, pursuant to that certain Continuing Contract Agreement for Professional Services
entered into as of March 15, 2005 ("Agreement'), by and between Indian River County, a
political subdivision of the State of Florida ("COUNTY") and Malcolm Pirnie, Inc.
("CONSULTANT').
The COUNTY has selected the CONSULTANT to perform the professional services
set forth on Attachment 1, attached to this Work Order and made part hereof by this
reference. The professional services will be performed by the CONSULTANT for the fee
schedule set forth in Attachment 2, attached to this Work Order and made a part hereof by
this reference. The CONSULTANT will perform the professional services within the
timeframe more particularly set forth in Attachment 3, attached to this Work Order and
made a part hereof by this reference, all in accordance with the terms and provisions set
forth in the Agreement. Pursuant to paragraph
