FOR
PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Carter Associates, Inc.
In accordance with Contract No, 0725
This Work Order No. 15 is in accordance with the existing AGREEMENT dated November 6tn
2007 between Carter Associates, Inc. (SURVEYOR) and Indian River County (COUNTY). This
Work Order No. 15 amends the agreement as follows:
SECTION I — PROJECT LIMITS
This Work Order No. 15 is for the SURVEYOR to perform a Boundary and Improvement
Location Survey of the parcel indicated below. The Survey is directly related to the 43`d Avenue
and State Road 60 Intersection Improvements project right-of-way acquisition applications.
Site is as follows:
Being property at the North West corner of 43`d Avenue and SR60 Intersection. "Marks Mobil"
Part of Tract 8 of Section 4-33-39, Indian River County, Florida
Parcel ID # 33390400001008000003.0
Address: 4310 201h Street, Vero Beach. FL. 32966

SECTION II - SCOPE OF SERVICES
Reference attached Exhibit A, proposal from Carter Associates, Inc.
As agreed upon between Carter Associates, Inc. and Indian River County, the SURVEYOR
shall provide Professional Land Surveying services to complete:
1. Boundary Survey of the parcel. Survey shall delineate the Boundary lines as per deed of
record; locate all above ground and apparent underground improvements. Underground
improvements that are not readily visible or obvious are not included in these tasks.
2. Survey shall depict all existing and proposed Rights -of -Way lines as per plans supplied
by Indian River County.
3. Project coordination.

SECTION III — TIME FOR COMPLETION & DELIVERABLEWWORK PRODUCT
1. Project shall be completed within thirty (30) business days from Notice to Proceed.
2. Deliverables SHALL BE CERTIFIED TO Indian River County and shall include five (5)
signed and sealed 24" x 36" AND three (3) reduced 8'/2" x 11" — "legible" versions (for
recording), electronic copies in AutoCAD 2007 and PDF formats.
SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as
identified in Sections I, II, and III of this Work Order No. 15, a lump sum fee of $3,100.00

The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein.

visible or obvious are not included in these tasks. Insert of the proposed 43rd Avenue and State
Road 60 Intersection Improvements project right-of-way acquisition applications.
Specific Notes:
a) All horizontal control shall be mapped in (1983 NAD, National Adjusted
Datum 1999 adjusted), Florida East Zone State Plane Grid Network.
b) Proposal does not include any public records searches for ownerships
and/or right of way takings.
COMPLETION & DELIVERABLESMORK PRODUCT:
