R. 512 Phase V, I-95 to 1301h Avenue (Willow St.) WORK ORDER NO . 3 (Engineering) FOR PROFESSIONAL SURVEYING SERVICES AGREEMENT FOR ANNUAL SURVEYING AND MAPPING SERVICES WITH Masteller, Moler & Reed, Inc. In accordance with Contract No . 0390 This Work Order No . 3 is in accordance with the existing AGREEMENT dated September 23 , 2003 , and the extension thereof, between Masteller, Moler & Reed, Inc . (SURVEYOR) and Indian River County (COUNTY) . This Work Order No . 3 amends the agreement as follows : SECTION I — PROJECT LIMITS County Road 512 , Phase V — the SURVEY LIMITS shall begin at the termination of County Road 512 Phase IV (East of I-95 ) and continue westerly on County Road 512 to 500 ' west of Willow Street ( 130th Avenue) SECTION II — SCOPE OF SERVICES Masteller, Moler & Reed, Inc . (SURVEYOR) shall supply field and office Professional Land Survey services to complete all task as outlined hereon in this work order No . 3 AND the attached (Exhibit "A") Task description outline . Brief: The scope of services for this Work Order No . 3 , is the completion of all task as described in exhibit "A" attached hereto, and preparation of a "Topographic" Survey for that portion of County Road 512 , Phase V as mentioned in section I. The survey will be prepared in accordance with the Chapter 177 & Chapter 472 Florida Statutes and Chapter 61G17 (Florida Minimum Technical Standards) as set forth for these types of surveys . SECTION III - TIME FOR COMPLETION An 80% submittal shall be delivered to the County for review and comments within 60 calendar days from Notice To Proceed (NTP) . Final submittal will be delivered to the County within 30 calendar days of receipt of the 80% submittal comments . Total 90 calendar days from NTP (Plus 80 % review time by the COUNTY) . SECTION IV — DELIVERABLES/WORK PRODUCT a) Survey shall consist of a cover sheet, survey horizontal and vertical control sheet an subsequent topographic sheets and details . b) Deliverables for the 80% submittal will consist of paper copies of the survey for review and comments . c) Deliverables for the final ( 100%) submittal to the County will consist of a digital file and three signed/sealed paper copies in 24" x 36 ' format . . It shall contain all information necessary for a third-party surveyor to independently recreate and/or utilize the survey work. It is acknowledged that all final products become property of Indian River County and will be available for use by the public at large . e) The SURVEYORS work product shall meet or exceed the minimum standards as defined by sections II, III, IV and exhibit "A" attached hereto and made part of this agreement, or the COUNTY will not approve the SURVEYORS request for payment. SECTION V - COMPENSATION The COUNTY agrees to pay , and the SURVEYOR agrees to accept, a fee for services rendered as identified in Section II, III and IV of this Work Order No . 3 , for a Lump Sum of $49 , 930 . 00 . All and/or any additional services not described hereon shall be pre -approved by the COUNTY . Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the Surveyor for the contract year of October 1 , 2004 through September 30 , 2005 . All invoicing and payments shall be accordance to original Professional Surveying Services Agreement for Annual Surveying and Mapping Services, contract No . 0390 with the COUNTY and as stated in sections III, IV and V hereon . The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein .
