WITH Masteller, Moler & Reed , Inc . Proposed 391h Avenue Between 69th Street and 71s ' Street Design/Topographic Survey IRC Project No . 0425 This Work Order No . 1 is in accordance with the existing AGREEMENT dated September 23 , 2003 between Masteller, Moler & Reed , Inc . (MMR) , Inc . (SURVEYOR) and Indian River County (COUNTY) . This Work Order No . 1 (Engineering) amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 1 (Engineering) is for providing all Professional Survey services related to a design/topographic survey, including but not limited to the research, field/office surveying and mapping of the proposed 39th Avenue Corridor. Limits shall include 69`h Street and 71 " Street easterly to the existing Railroad grade as specified in attached scope of services . Project Lying in the S . W. portion of Section 3 , TWP32 , RGE39 . SECTION II — SCOPE OF SERVICES Supply all field and office services and deliverables to complete the mentioned tasks as denoted in attached scope of services supplied by MMR (exhibit A) , dated April 29 , 2004. The scope of services shall be followed to accomplish all tasks and results as anticipated by the County. Any changes and/or modification of said scope of services shall be pre-approved in writing by the County prior to proceeding. SECTION III - TIME FOR COMPLETION/SUBMITTALS All tasks shall be completed, in its entirety within 60 calendar days of notice to proceed . A +/- 80 % preliminary survey submittal shall be submitted for review and comments by the County . All final submittals and revisions shall be complete at the end of the term unless pre -approved by the County in writing . The County shall be included in any meetings and/or outside consultations pertaining to the outcome of said project . All invoicing shall be supported with documentation (Man hours) of time and materials spent prior to payments . The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein . Dear Mr. O ' Brien: We thank you for the opportunity to offer the expertise and experience of Masteller, Moler & Reed, Inc . relative to the proposed referenced project. Upon field investigation of the site, Masteller, Moler & Reed, Inc. proposes to furnish the following: Perform a route survey for the purpose of design of a roadway. The project area is as follows : 1 ) 600 feet East & West of the proposed centerline of the 39th Avenue corridor along 69th Street. 2) 600 feet West of the proposed centerline of the 39th Avenue corridor and east to the East right-of--way line of the Old Dixie highway along 71st Street. 3 ) The proposed 39th Avenue corridor (200 ' wide) located between 69th and 71s' Street per documentation as provided by the County. Generally the limits of the survey within the project area are as follows . 4) 50 feet North & South of the right-of-way lines of 71 " Street along the project area as described in paragraph #2 above . 5) 50 feet North & South of the right-of-way lines of 69th Street along the project area as described in paragraph # 1 above . 6) 100 feet each side of the proposed centerline of the 39th Avenue corridor. The scope of services to be provided for this project will be as follows : A) Establish horizontal baseline control (or reference to) tied to sectional and/or established Boundary/Parcel lines. Control shall be set as to allow for reestablishment for construction purposes in the future. B) Establish in the field, control point ' s minimum 500 ' intervals . (State Plane NAD83190) C) Establish vertical control at minimum 500 ' intervals . Coordination with I.R. C . utilities and plotting of any exiting utilities to be included in this task. E) Trees not part of this scope unless aged large oaks and/or trees of possible concern. F) Delineate any areas of environmental concern. If areas exist I.R. C . shall mark areas as needed. (None expected) G) Cross-sections at 100 ' intervals or less as needed to determine terrain and/or features . Cross-section shall extend at least 50 ' beyond existing and proposed right-of--way line or greater as necessary to define grade. H) Any additional task as to supply all necessary information for design purposes not outlined hereon. Deliverables : To be used for design and records . A) 1 -24" x 36" certified sheet plans including cover, control sheet, topographic/cross- section and profiles at 100 ' intervals for design. B) 3 - 11 "x14 certified versions . C) File on disc. AutoCad 2001 or comparable (State Plane) . County Will Provide : (As needed) A) Recent Boundary survey (2003 ) of North portion (Hatala property) B) NGVD29 benchmarks in area. C) Existing Sketch and Description of acquired (Or to be acquired) corridor through private lands. D) GPS/State Plane control within a mile of site. Nevertheless, no representations or warranties are made as to the success, approval or . the issuance of permits on any application submitted by Owner/Client based in whole or in part upon the surveys prepared by Masteller, Moler & Reed, Inc. Masteller, Moler & Reed, Inc. will not accept backcharges unless we provide a written agreement covering all corrective action and the total amount of the backcharge necessary to accomplish the corrective action. d) Print Costs & Reimbursables Blueprints (24" x 36") $ 1 . 00/each Concrete Monuments $ 9 . 00/each e) Price Guarantee Prices quoted are firm for sixty (60) days from the date of this proposal . fl Contract Assignment While binding upon the parties, their successors or assigns, this Contract may not be transferred or assigned without the consent of both parties. The Owner/Client reserves the right to terminate this Contract within ten ( 10) days written notice. Masteller, Moler & Reed, Inc. shall be compensated for services performed to termination date.
