PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Masteller, Moler, Reed & Taylor, Inc.

This Work Order No. 7 is in accordance with the existing AGREEMENT dated November 6"',
2007 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County
(COUNTY). This Work Order No. 7 amends the agreement as follows:
SECTION I — PROJECT LIMITS
This Work Order No. 7 is for the SURVEYOR to perform additional services in connection with
the Right of Way Mapping of County Road 512 Phase 3. County Road 512 Roadway
Improvements whose limits range from County Road 510 (Sta. 274+00) to Roseland Road (Sta.
346+00), all lying in Indian River County, Florida. (7200 LF). Limits include all properties
associated with the construction project.
SECTION II - SCOPE OF SERVICES
As agreed upon between Masteller, Moler, Reed & Taylor, Inc. and Indian River County, the
SURVEYOR shall provide Professional Land Surveying services to complete all tasks as
outlined in this Work Order No. 7 described as follows:
1. Project (as a minimum) shall adhere to attached Exhibit A "Proposal" and Right -of -
Way Map "Checklist" Exhibit B.
2. Project initiation and research, including review of existing files and field notes,
preparation of work orders and field crew folders.
3. Perform reconnaissance within the project limits. Recover existing monumentation
to establish existing R/W lines, property lines, etc. Perform tie-in of existing
monumentation including State Plane coordinates.
4. Coordinate with disk supplier with regards to stamping including review of samples
and final ordering.

5. Set Permanent Reference Monuments along the Right of Way at all changes in
direction, including all Points of Curvature and Tangency. Provide State Plane
Coordinate values on all monumentation set. PRM's shall be an iron rod and disk
stamped as agreed and encased in a 4" by 4" concrete monument and shall be
placed at a maximum of 1000 feet apart.
6. Set Permanent Reference Monuments along related project properties as applicable.
PRM's shall be an iron rod and disk stamped as agreed and encased in a 4" by 4"
concrete monument and shall be placed at a maximum of 1000 feet apart.
7. Monument all Permanent Control Points (PCP's) along the baseline at all changes in
direction, including all Points of Curvature and Tangency. Provide State Plane
Coordinate values on all baseline control points. PCP's shall be a Parker Kaylon nail
and tab stamped as agreed and shall be placed at a maximum of 1000 feet apart.
8. Examine and review updated title report for twenty seven adjacent properties as
necessary. Include revisions on final map as necessary. Revise and update existing
Right of Way Map to depict all set or found monumentation and current title work as
applicable. Map shall adhere to attached Right of Way Map checklist (Exhibit "B").
Provide copies for review and comment. Address comments and prepare mylars as
required.
SECTION III — TIME FOR COMPLETION & DELIVERABLESIWORK PRODUCT
1. Project shall be completed as follows:
a. Time of 80% - 90% PRELIMINARY review submittal shall be 60 calendar Days from
notice to proceed.
b. Time of FINAL project completion shall within 30 calendar days of receipt of the
County's review comments.
2. Deliverables -The SURVEYOR shall provide the COUNTY:
a. PRELIMINARY REVIEW. 80% - 90% product for County review and comments. One
(1) paper "hardcopy". County shall strive to provide review comments within 30
calendar days of preliminary submittal.
b. FINAL one (2) paper "hardcopy" signed and sealed sets and one (1) signed and
sealed MYLAR sets in adequate recording fashion (I.E. Borders). Sheeted and
model space (as applicable) AutoCad drawing file and PDF formats on CD.
c. Survey set/sheets shall include a cover sheet with location sketch, survey
certifications, and related title and project No; a survey notes/legend/abbreviation
sheet, survey horizontal control sheet(s), right-of-way plan view sheets and/or as
indicated in the County supplied Right -of -Way map "checklist".
d. Work Product and digital versions are to prepared and submitted so that the
COUNTY or other consultants can readily use it for the design and analysis of the
area, as defined. It shall contain all information necessary for third -party surveyor to
independently recreate and/or utilize the survey work. It is acknowledged all final
products become property of Indian River COUNTY and will be available for use by
the public at large.
a. The SURVEYOR'S work product shall meet or exceed the minimum standards as
defined by Sections II, III and IV or the COUNTY will not approve the SURVEYOR'S
request for payment.

SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for the above described
services rendered as identified in Sections I, II, and III of this Work Order No. 7, for a total lump
sum fee of $15,200.00.
All and/or any additional services not described hereon shall be pre -approved by the
COUNTY. Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year(s) of November 6th, 2007
through November 5th, 2009.
All invoicing shall include project identification, Work Order No. (WO 7), Contract Number 0725,
itemize man-hours and materials expended to complete the scope of services. Payments shall
be in accordance with the original Professional Surveying and Mapping/GIS Services
Agreement, contract No: 0725 with the COUNTY and as stated in Section II, III and IV hereon.

The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein.
IN WITNESS WHEREOF the parties hereto have executed these presents this 9th day
of Sept. 2008.

Hereby requests and authorizes the following services:
Perform RNV Monumentation Map for property located in Indian River County and identified as County Road 512 Phase 3
from Roseland Road to County Road 510 and includes the following Scope of Services:
1. Project initiation and research, including review of existing files and field notes, preparation of work orders and field
crew folders.
2. Perform reconnaissance within the project limits. Recover existing monumentation to establish existing R/W lines,
property lines, etc. Perform tie-in of existing monumentation including State Plane coordinates.
3. Coordinate with disk supplier with regards to stamping including review of samples and final ordering.

8. Provide five (5) certified copies and a digital file of the survey and prepare its work product digital versions so that the
County or other consultants can readily use it for the design and analysis of the area, as defined.
We propose to perform the above described scope of services for a lump sum fee of $ 15,200.00.
Additional charges may also apply for research and office work and expenses necessary to resolve problems with your
legal description, conflicts with neighboring legal descriptions, and/or discrepancies due to adverse use, inaccurate or
missing information.

INDIAN RIVER COUNTY
RIGHT OF WAY MAP CHECKLIST
All Right-of-way projects shall adhere to Chapter 177 & 472, F.S. and Chapter 61G17,
F.A.C. as minimum and be certified as such.
R/W Maps shall be prepared/submitted in the form of a certified R/W Map will all
applicable certifications and criteria met.
1. Cover sheet/Key sheet. (To be supplied by IRC in DWG.)
2. Survey notes and report sheet.
3. Key sheet(s)
4. Survey Control sheet.
5. Subsequent R/W map sheets.
6. Supply a Box or area for county acceptance and/or certification.
7. Title Box of each sheet must contain:
a. County
b. State Road and/or County Road Number
C. Designed, drawn and checked by (use names or initials) & date
d. Scale
e. Field Book References
f. Sheet Numbers to be in numerical order (1, 21 31 4, etc.) NOT (1, 1A, 1 B, 2, etc.)
8. Any notes necessary to set out the type and/or purpose of the map shall be shown.
9. Provide in survey notes - all relevant resources to prepare the Right -of -Way map.
10. Show pertinent dimensions and bearings in the following fashion as the case may be:
a. Plat — (P)
b. Computed — (C)
C. Field — (F)
d. Deed (D)
11. Table of Ownerships shall show:
a. Parcel Number
b. Recording Data
12. Show stations and offsets to all Right of Way Breaks, bends, PC's, PT's, block corners (per plat).
13. Show stations, deflection (or bearing) of all intersecting section, % section lines to baseline (either
field or calculated as applicable).
14. Show monumented baselines and all references thereto.
15. Verify that computed distances and bearings are supported by field measurements.
16. Verify and/or monument significant elements of Baseline alignment shown.(i.e. curves, bearings
and P.I. Stations)
17. Ensure that significant elements of all curves are shown and are correct. Also show curve data on
all sheets where they appear.
18. Provide stations on the controlling baseline at the beginning, ending of R/W Project, 500' intervals,
all w bends, PC's, PT's, Pl's.
19. Stationing in field minimum 500' beyond beginning and ending of project.
20. Baseline (equate to intersecting State or County Jobs by Station and job number when available)
as available.
21. Projects shall be in NAD83/90 State plane coordinate system and bearings reference shall be
shown and must be clearly stated. Values shall be place on key baseline monumentation.

acquisition or old title references — use system of dashed lines, dotted figures, etc., with ties shown
and name and plat book reference indicated
40. Stations and Angle Ties to ALL Existing Major and Minor Side Streets (Public Road R/W) as
surveyed and/or calculated.
41. Show field located P. R. M. /major property corner or parcel ties by station/offset to baseline/control
line.

43. Any buildings within 25 feet of proposed R/W line shall be dimensioned to R/W line. If proposed
R/W line is unknown, obtain buildings within +/- 25' of existing R/W. Locations of buildings and
large structures can be obtained from field and/or photographic / planimetric methods, note shall be
placed on plan as to origin.
44. Show and identify all easements within, along or abutting the Right -of -Way or affected property on
detail Maps by width, use and ownership (where applicable).
45. Any other items as to meet the goals and objectives established by the County.
