In accordance with Contract No . 0725 This Work Order No . 9 is in accordance with the existing AGREEMENT dated November 6 , 2007 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County (COUNTY) . This Work Order No . 9 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 9 is for the SURVEYOR to perform additional services in connection with the County Road 512 Phase III - Pond E , St . Johns River Water Management District Conservation Easement and permitting requirements . SECTION II - SCOPE OF SERVICES As agreed upon between Masteller, Moler, Reed & Taylor, Inc. and Indian River County , the SURVEYOR shall provide Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 9 described within the attached Masteller, Moler, Reed and Taylor Inc "Agreement for Professional Services" and as follows : 1 . Perform field and office professional Surveying and Mapping services related to completion of a Boundary and Improvement Survey ( Easement Boundary limits to be determined by the County) . Project to meet or exceed the St . Johns River Water Management District permitting requirements . Project shall be completed as follows : a . 15 days from Notice to Proceed AND/OR from time of Boundary limits re- configuration is supplied by the County . 2 . Deliverables -The SURVEYOR shall provide the COUNTY : a . FINAL three (3) paper " hardcopy" signed and sealed AutoCad drawing file and PDF formats on CD . b. Work Product and digital versions are to prepared and submitted so that the COUNTY or other consultants can readily use it for the design and analysis of the area , as defined . It shall contain all information necessary for third-party surveyor to independently recreate and/or utilize the survey work . It is acknowledged all final products become property of Indian River COUNTY and will be available for use by the public at large . a. The SURVEYOR' S work product shall meet or exceed the minimum standards as defined by Sections 11 , 111 and IV or the COUNTY will not approve the SURVEYOR' S request for payment . SECTION IV — COMPENSATION The COUNTY agrees to pay , and the SURVEYOR agrees to accept , for the above described services rendered as identified in Sections 1 , II , and III of this Work Order No . 9 , for a total lump sum fee of $4 , 380 . 00 All and/or any additional services not described hereon shall be pre-approved by the COUNTY . Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule . All invoicing shall include project identification , Work Order No . (WO 9) , Contract Number 0725 , itemized man-hours and materials expended to complete the Scope of Services . Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services Agreement , contract No : 0725 with the COUNTY and as stated in Section 11 , III and IV hereon . This Work Order No. 9 :mends the agreement as follows: Perform Boundary Survey for the Conservation Easement area located in Indian River County and lying in Section 23 , Township 031 South, Range 038 East and being a portion of Tax Parcel 31382300000500000001 .2 and includes the following Scope of Services: 1 . Perform boundary survey of Conservation Easement as described in sketch and legal description prepared by David Silon, dated 7- 11 -07 as provided by the County. Boundary survey shall be prepared in accordance with the Minimum Technical Standards for surveying as adopted by the State of Florida. 2. Locate and depict existing improvements including walking path and fencing. 3_ Monument all property comers as required and identified in St. Johns River Water Management District permit #56415-2, condition #2. 4 . Prepare Map of Survey as required, depicting improvements, adjacent uses and monumentation set. 5. Certify as requested, provide ten copies and one compact disk containing the AutoCAD and .pdf digital file of the survey sketch. We propose to perform the above described scope of services for a lump sum fee of S 4,380.W. We expect the time of completion to be 15 days from the notice to proceed . Upon your verbal authorization for this survey, we order research materials and set up your job file. In the event of a cancellation and in order to cover our costs, you will be billed for all work/expenses incurred up to the date of cancellation, including costs to close out your file. See the attached for standard conditions. All and/or any additional services not described hereon shall be pre-approved by the COUNTY. Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the SURVEYOR for the contract year(s) of November 6th2007 through November 5th, 2009.
