Home My WebLink About2004-126 0 NON-EXCLUSIVE PROFESSIONAL SURVEYING AGREEMENT CONTINUING SURVEYING SERVICES WORK AUTHORIZATION # 1 West Wabasso Proposed Waterline Project • Scope of Services Masteller, Moler & Reed, Inc . will provide for a route survey for engineering design purposes for a proposed utility pipeline along the following route . 1 ) 63 `d Avenue — From C .R. 510 north to 87th Place . 2) 86th Place — From 63 `d Avenue to 640 Avenue . 3 ) 87th Street — From 61 " Drive, west to a proposed utility easement located f 300 feet west of 64th Avenue . 4) 87th Place — From 63 `d Avenue to 64th Avenue . 5 ) Proposed utility easement — From 87th Street, north 1300 feet, then east to 64th Avenue. 6) 58th Court — From C . R. 510 south to terminus . 7) 59th Avenue — From C .R. 510 south to terminus. • Th approximate project length being 7400 feet in length. The route survey includes right of way location and locations of all visible objects (IE : Power poles, mailboxes, driveways, etc . ) within the proposed routes and will be prepared at a scale as directed by the engineer. Elevations will be based on NGVD 1929 and x-sections will be obtained at 200 foot intervals along the project route . The project will also be tied into the Indian River County horizontal network system (State Plane Coordinates) . • PLEASE NOTE THAT IF EASEMENT DOCUMENTS NEED TO BE PREPARED, WE WILL INVOICE AT OUR STANDARD EASEMENT DOCUMENT RATE OF $400.00 PER DOCUMENT. THIS ITEM IS NOT INCLUDED IN OUR LUMP SUM FEE. Upon completion (5 ) copies and ( 1 ) digital file will be provided. 1 of 4 ` Work Authorization # 1 05/21 /04 Page 2 • Consultiniz Surveying Insurance Requirements The SURVEYOR shall, at least ten ( 10) days prior to the commencement of any work covered by a specific approved Work Order, provide to the COUNTY a certificate of commercial general liability insurance with a reputable insurance company authorized to do business in the State of Florida, subject to approval by the COUNTY' s risk manager, in an amount not less than $ 1 , 000,000 combined single limit for bodily injury and property damage, including coverage for premises/operations, products/completed operations, contractual liability, and independent contractors, in accordance with the COUNTY' s Administrative Policy Manual . The SURVEYOR shall, at least ten ( 10) days prior to the commencement of any work covered by a specific approved Work Order, provide to the COUNTY a certificate of professional liability insurance with a reputable insurance company authorized to do business in the State of Florida, subject to approval by the COUNTY ' s risk manager, in an amount not less than $ 1 , 000,000 . The SURVEYOR shall, at least ten ( 10) days prior to commencement of any work covered by a specific approved Work Order, provide to the COUNTY a certificate of business auto liability insurance authorized to do business in the State of Florida, subject to approval by the COUNTY ' s risk manager, in an amount not less than $ 1 , 000,000 per occurrence combined single limit for bodily injury and property damage, including coverage for owned autos and other vehicles, hired autos and other vehicles, non-owned autos and other vehicles, in accordance with the COUNTY ' s Administrative Policy Manual . The commercial general liability and auto liability insurance policies shall name Indian River County, a political subdivision of the State of Florida, as an additional insured. In addition, the SURVEYOR shall , at least ten ( 10) days prior to the commencement of any work covered by a specific Work Order, provide to the COUNTY a certificate of worker ' s compensation insurance , including employer ' s liability, with a reputable insurance company authorized to do business in the State of Florida, subject to approval by the COUNTY' s risk manager, with a limit of $ 100,000 per accident, $ 500,000 disease (policy limit) , and a $ 100 ,000 disease (each employee) in compliance with all state and federal laws, and in accordance with the COUNTY ' s Administrative Policy Manual . The SURVEYOR shall provide to the COUNTY at least thirty (30) days written notice by registered mail , return receipt requested, addressed to the COUNTY' s risk manager, prior to cancellation or modification of any required insurance. SURVEYOR shall cause any subcontractor performing any work to provide to COUNTY certificates of insurance under the same conditions and with the same policy limits as required of the SURVEYOR. • Method and Amount of Compensation Masteller, Moler & Reed, Inc . proposes to provide for the above Scope of Services based on the following lump sum fee : 2 of 4 Work Authorization # 1 05/21 /04 Page 3 Preparation of survey base plans for the purpose of Engineering design of utility pipeline to be constructed at West Wabasso $ 10,300 . 00 • PLEASE NOTE THAT IF EASEMENT DOCUMENTS NEED TO BE PREPARED, WE WILL INVOICE AT OUR STANDARD EASEMENT DOCUMENT RATE OF $400.00 PER DOCUMENT. THIS ITEM IS NOT INCLUDED IN OUR LUMP SUM FEE. • Additional Work Performed Any and all work performed, other than that specifically contracted for within this general contract, shall be billed at the following job classification and rates : Professional Surveyor (Principal) $ 120 . 00/hr. Professional Surveyor (P . S . M. ) $ 95 . 00/hr. Survey Project Manager (P . S .M. ) $ 85 . 00/hr. Project Surveyor (Unregistered) $ 75 . 00/hr. Three-Man Field Crew $ 115 . 00/hr. Two-Man GPS Crew $ 115 . 00/hr. Three-Man GPS Crew $ 125 . 00/hr.
