ANNUAL SURVEYING and MAPPING/GIS SERVICES

This Work Order No. 8 is in accordance with the existing AGREEMENT dated November 6',
2007 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County
(COUNTY). This Work Order No. 8 amends the agreement as follows:
SECTION I — PROJECT LIMITS
This Work Order No. 8 is for the SURVEYOR to perform reconnaissance with project limits to
identify areas of high water. Areas of concern include canals, ditches, lakes and swales.
Project limits are from 12th Street to 53rd Street and from 82nd Avenue to the Indian River.
SECTION II - SCOPE OF SERVICES
As agreed upon between Masteller, Moler, Reed & Taylor, Inc. and Indian River County, the
SURVEYOR shall provide Professional Land Surveying services to complete all tasks as
outlined in this Work Order No. 8 described as follows:
1. Project preparation, including work orders and field crew folders. Coordinate with
County staff to establish guidelines.
2. Perform reconnaissance with project limits to identify areas of high water. Areas of
concern include canals, ditches, lakes and swales. Record area on field notes and
provide photograph of affected area. Locate High Water Mark and record GPS Real
Time horizontal location and vertical elevation of water mark and record in field
notes.
3. State Plane Coordinates shall be based upon the North American Datum, 1983/1990
adjustment, Florida East Zone. Elevations shall be based upon the National
Geodetic Vertical Datum of 1929.

4. Prepare deliverables, including organizing pictures, field notes, ASCII file and
Surveyor's report.
SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT
1. Project shall be completed as follows:
a) Project completed 30 days from Notice to Proceed, on an as needed basis.
2. Deliverables -The SURVEYOR shall provide the COUNTY.
a) An ASCII file containing the northing, easting, elevation and description of the
points collected.
b) A signed and sealed Surveyor's Report documenting the methods of data
collection, time of survey and other items as determined by the County or the
Surveyor along with a CD containing pictures and ASCII File.
c) Work Product and digital versions are to prepared and submitted so that the
COUNTY or other consultants can readily use it for the design and analysis of
the area, as defined. It shall contain all information necessary for third -party
surveyor to independently recreate and/or utilize the survey work. It is
acknowledged all final products become property of Indian River COUNTY
and will be available for use by the public at large.
d) The SURVEYOR'S work product shall meet or exceed the minimum
standards as defined by Sections II, III and IV or the COUNTY will not
approve the SURVEYOR'S request for payment.
SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for the above described
services rendered as identified in Sections I, II, and III of this Work Order No. 8, for a total lump
sum fee of $3,485.00.
All and/or any additional services not described hereon shall be pre -approved by the
COUNTY. Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year(s) of November 6th, 2007
through November 5th, 2009.
All invoicing shall include project identification, Work Order No. (WO 8), Contract Number 0725,
itemize man-hours and materials expended to complete the scope of services. Payments shall
be in accordance with the original Professional Surveying and Mapping/GIS Services
Agreement, contract No: 0725 with the COUNTY and as stated in Section II, III and IV hereon.

The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein.
IN WITNESS WHER OF'ihe parties hereto have executed these presents this 14th day

Hereby requests and authorizes the following services:
Perform Specific Purpose Survey for property located in Indian River County and identified as the Tropical
Storm Fay related High Water Mark Survey and includes the following Scope of Services:
1. Project preparation, including work orders and field crew folders. Coordinate with County staff to
establish guidelines.
2. Perform reconnaissance with project limits to identify areas of high water. Areas of concern include
canals, ditches, lakes and swales. Record area on field notes and provide photograph of affected
area. Locate High Water Mark and record GPS Real time horizontal location and vertical elevation
of water mark and record in field notes.

We propose to perform the above described scope of services for a lump sum fee of $ 31485.00.
Upon your verbal authorization for this survey, we order research materials and set up your job file. In the
event of a cancellation and in order to cover our costs, you will be billed for all work/expenses incurred up to
the date of cancellation, including costs to close out your file. See the attached for standard conditions.
Price is based and contingent upon site conditions described by Client or Clients representative. If, upon
arrival at job site, conditions described do not accurately reflect the actual conditions of the property, charges
for additional work may apply.
Additional charges may also apply for research and office work and expenses necessary to resolve problems
with your legal description, conflicts with neighboring legal descriptions, and/or discrepancies due to adverse
use, inaccurate or missing information.
