In accordance with Contract No . 0725 This Work Order No . 10 is in accordance with the existing AGREEMENT dated November 6 , 2007 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County (COUNTY) . This Work Order No . 10 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 10 is for the SURVEYOR to perform professional Surveying and Mapping services in connection with the 66"' Avenue Road-Way Improvements Project , Right-of-Way Acquisition applications . SECTION 11 - SCOPE OF SERVICES As agreed upon between Masteller, Moler, Reed & Taylor, Inc . and Indian River County , the SURVEYOR shall provide Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 10 and described within the attached Masteller, Moler, Reed and Taylor, Inc. "Agreement for Professional Services" as follows : Perform field and office professional Surveying and Mapping services related to completion of a Boundary and Improvement Surveys of the four (4) parcels identified in items one , two , three and four. Reference detailed Scope of Services in Attached Exhibit A. 1 . Boundary Improvement Survey for: ❑ Marotto Parcel , 5215 66th Avenue, Right-of-Way Parcel No. 106 (Arcadia Plans) Approximately 2 . 0 acre property located in Indian River County and lying in Section 19 , Township 032 South , Range 039 East and identified as Tax Parcel 32-39- 19-00001 -0080- 00003 . 0 2 , Boundary Improvement Survey for: ❑ Lewis Parcel , 5265 66n' Avenue, Right-of-Way Parcel No. 105 (Arcadis Plans) Approximately 2 . 0 acre property located in Indian River County and lying in Section 19, Township 032 South , Range 039 East and identified as Tax Parcel 32-39- 19-00001 -0080- 00002 . 0 IRC Work Order # 10 66Ave — Right-of-Way Acquisition Four Bnd,M Surveys January , 2009 Agenda Page 2 of 4 3 . Boundary Improvement Survey for: ❑ Harris Groves, 61st Street and 66th Avenue, Right-of-Way Parcel No. 102 (Kimley Horn Plans) Approximately 37 acre property located in Indian River County and lying in Section 18 , Township 032 South , Range 039 East and identified as Tax Parcel 32-39- 18-00001 -0010- 00001 . 0 SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT 1 . Project shall be completed as follows : a . 30 days from Notice to Proceed . 2 . Deliverables -The SURVEYOR shall provide the COUNTY . a . FINAL five (5) 24" x 36" paper signed and sealed " hardcopy" , AutoCad drawing file and PDF formats on CD . b . Work Product shall be prepared and supplied in State Plane geometry ( NAD83 2007 adjustment if applicable) ; digital versions are to be prepared and submitted so that the COUNTY or other consultants can readily use it for the design and analysis of the area , as defined . It shall contain all information necessary for third-party surveyor to independently recreate and/or utilize the survey work . It is acknowledged all final products become property of Indian River COUNTY and will be available for use by the public at large . a . The SURVEYOR' S work product shall meet or exceed the minimum standards as defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR' S request for payment . Remainder of page left blank intentionally IRC Work Order #10 66Ave — Right-of-Way Acquisition Four Bndr Surveys January 6 , 2009 Agenda Page 3 of 4 SECTION IV — COMPENSATION The COUNTY agrees to pay , and the SURVEYOR agrees to accept , for the above described services rendered as identified in Sections I , II , and III of this Work Order No . 10 , for a total lump sum fee of: Item One , Marotto Parcel - $3 , 900 . 00 Item Two , Lewis Parcel - $3 , 900 . 00 Item Three , Abazis Parcel - $4 , 400 . 00 Item Four, Harris Groves - $4 , 800 . 00 Total Lump Sum - $ 17 , 000 . 00 All and/or any additional services not described hereon shall be pre-approved by the COUNTY. Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule . All invoicing shall include project identification , Work Order No . (WO 10) , Contract Number 0725 , itemized man -hours and materials expended to complete the Scope of Services . Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services Agreement , contract No : 0725 with the COUNTY and as stated in Section 11 , III and IV hereon . All remaining sections of the AGREEMENT shall remain in full force and effect , and are incorporated herein . We propose to perform the above described scope of services for a lump sum fee of $ 3 , 900 . 00 . We estimate the time of completion to be 3 weeks from the notice to proceed . Upon your verbal authorization for this survey, we order research materials and setup your job file . In the event of a cancellation and in order to cover our costs, you will be billed for all work/expenses incurred up to the date of cancellation , including costs to close out your file .
