In accordance with Contract No , 0725 2007 -2009 This Work Order No . 11 is in accordance with the existing AGREEMENT dated November 61" 2007 between Masteller , Moler , Reed & Taylor , Inc . ( SURVEYOR ) and Indian River County ( COUNTY) . This Work Order No . 11 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 11 is for the SURVEYOR to perform surveying services in connection with the Right of Way Mapping of County Road 512 , Phase IV Roadway Improvements whose limits range from Interstate 95 ( Sta . 35+ 00 ) to 1 , 000 feet west of County Road 510 ( Sta . 152 + 00 & commencement point for existing Phase III R/W ) , all lying in Indian River County , Florida . Length is approximately 13 , 500 feet ( 2 . 6 miles ) , limits include all properties associated with the construction project . SECTION II - SCOPE OF SERVICES As agreed upon between Masteller , Moler , Reed & Taylor , Inc . and Indian River County , the SURVEYOR shall provide Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 11 described as follows : i . Project initiation , including review of existing files and field notes , preparation of work orders and field crew folders . 2 . Project research including compilation of deeds , legal description and right of way purchases and takings . 3 . Research and review property tax records for adjacent properties as necessary . Depict provided documentation and current owner on Right of Way Map . 4 . Perform reconnaissance within the project limits . Recover existing monumentation to establish existing R/W lines , property lines , etc . Set Permanent Reference Monuments along the Right of Way at all changes in direction , including all Points of Curvature and Tangency . Provide State Plane Coordinate values on all monumentation set . PRM ' s shall be an iron rod and disk stamped as agreed and encased in a 4" by 4" concrete monument and shall be placed at a maximum of 1000 feet apart . 6 . Set Permanent Reference Monuments along related project stormwater pond properties as applicable . PRM 's shall be an iron rod and disk stamped as agreed and encased in a 4" by 4" concrete monument and shall be placed at a maximum of 1 , 000 feet apart . 7. Monument all Permanent Control Points ( PCP's) along the baseline at all changes in direction , including all Points of Curvature and Tangency . Provide State Plane Coordinate values on all baseline control points . PCP 's shall be a Parker Kaylon nail and tab or a 4" by 4" concrete monument stamped as agreed and shall be placed at a maximum of 1 , 000 feet apart . 8 . Prepare Right of Way Map to depict all set or found monumentation and current title work as applicable . Map shall adhere to Indian River County Right of Way Map checklist. Provide copies for review and comment. 9 . Address comments and prepare mylars and other deliverables as required . 10 . Project (as a minimum ) shall adhere to attached Exhibit A " Proposal " and Right-of- Way Map "Checklist" Exhibit B . SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT 1 . Project shall be completed as follows . a. Time of 90% PRELIMINARY review submittal shall be 90 calendar Days from notice to proceed . b . Time of FINAL project completion shall be within 30 calendar days of receipt of the COUNTY review comments . 2 . Deliverables -The SURVEYOR shall provide the COUNTY : a . PRELIMINARY REVIEW . 90% product for COUNTY review and comments . One ( 1 ) paper "hardcopy" and DWF formatted digital file . COUNTY shall strive to provide review comments within 30 calendar days of preliminary submittal . b. FINAL two (2) paper " hardcopy" signed and sealed sets and one ( 1 ) signed and sealed MYLAR sets in adequate recording fashion ( I . E . Borders ) . Sheeted and model space ( as applicable ) AutoCad drawing file and PDF and DWF formats on CD . c. Survey set/sheets shall include a cover sheet with location sketch , survey certifications , and related title and project number ; a survey notes/legend/abbreviation sheet , survey horizontal control sheet(s ) , right-of- way plan view sheets and/or as indicated in the COUNTY supplied Right- of-Way map "checklist" . d . Work Product and digital versions are to prepared and submitted so that the COUNTY or other consultants can readily use it for the design and analysis of the area, as defined . It shall contain all information necessary for third-party surveyor to independently recreate and/or utilize the survey work . It is acknowledged all final products become property of COUNTY and will be available for use by the public at large . e . The SURVEYOR ' S work product shall meet or exceed the minimum standards as defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR' S request for payment. MEMMONNOM IRC Work Order #11 County Road 512 Ph . 4 — R/W monumentation Page 3 of 9 SECTION IV — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept , for the above described services rendered as identified in Sections I , II , and III of this Work Order No . 11 for a total lump sum fee of $32 , 700 . 00 . All and/or any additional services not described hereon shall be pre - approved by the COUNTY. Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the SURVEYOR for the contract year (s ) of November 6th , 2007 through November 5'h , 2009 . All invoicing shall include project identification , Work Order No . (WO 11 ) , Contract Number 0725 , itemize man - hours and materials expended to complete the scope of services . Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services Agreement, contract No : 0725 with the COUNTY and as stated in Section II , III and IV hereon . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein . S. and Chapter 61G17, F . A. C. as minimum and be certified as such . R/W Maps shall be prepared/submitted in the form of a certified R/W Map will all applicable certifications and criteria met. A Copy of this check list is to be provided to the county with all submittals . The check list items shall be individually noted as complete, pending etc with a note explaining why the item is not competed. 1 . Cover sheet/Key sheet . (To be supplied by IRC in DWG . ) 2 . Survey notes and report sheet . 3 . Key sheet ( s ) 4 . Survey Control sheet . 5 . Subsequent R/W map sheets . 6 . Supply a Box or area for county acceptance and/or certification . 7 . Title Box of each sheet must contain : a . County b . State Road and/or County Road Number c . Designed , drawn and checked by ( use names or initials ) & date d . Scale e . Field Book References f . Show any replat of a subdivision — show old subdivision when necessary to clear up former acquisition or old title references — use system of dashed lines , dotted figures , etc. , with ties shown and name and plat book reference indicated 40 . Stations and Angle Ties to ALL Existing Major and Minor Side Streets ( Public Road R/W) as surveyed and/or calculated . 41 . Show field located P . R . M . /major property corner or parcel ties by station/offset to baseline/control line . 42 . Current Street , County or State Road names and historical names are to be provided . 43 . Any buildings within 25 feet of proposed R/W line shall be dimensioned to R/W line . If proposed R/W line is unknown , obtain buildings within +/- 25' of existing R/W . Locations of buildings and large structures can be obtained from field and/or photographic / planimetric methods , note shall be placed on plan as to origin . 44 . Show and identify all easements within , along or abutting the Right-of-Way or affected property on detail Maps by width , use and ownership (where applicable) . 45 . Monumentation shall be a standard 4" x 4" ( PRM) concrete monument with Indian River County Disc. All Disc are to be stamped Right- of-Way (or abbreviation ) and with the surveyors LB or License Number 46 . Permanent Control Points ( PCP) shall be a standard PK nail (or substitute) with an Indian River County Disc. All Disc are to be stamped Base- Line or reference line (or abbreviation) as applicable and with the surveyors LB or License Number 47 . Any other items as to meet the goals and objectives established by the County.
