Between the Owner:
Board of County Commissioners, Indian River County, Florida
and the Architect:
Donadio & Associates, Architects of Vero Beach, Florida
For the Project:
Indian River County New Administration Complex to be generally located on approximately 13 acres north of
26th) Street. east of 20th Avenue. south of Aviation Boulevard and west of Lateral "E" Canal in Vero Beach.
Florida. This Project shall also include but not be limited to other such improvements as the Extension of 20th
Avenue to the North and Construction of a Bridge across the Main Relief Canal connecting 20th Avenue with
Aviation Boulevard.
AMENDMENT NUMBER 4 :
Authorization is granted to proceed with payment of professional fees for Additional Services provided as
described in Exhibit A attached to this Amendment Number 4.
As Follows:
Redesign of a portion of former Supervisor of Elections in Building #2 for use as a Conference Room —
Additional Architectural/Engineering Fee — $8,500 (for all related services)
Compensation :
The architect's compensation for all services under the Primary Contract including the previous 3 Amendments
is currently 52,361 ,800 (Two Million, Three Hundred Sixty One Thousand, Eight Hundred Dollars and No
Cents)
Total increase in professional fees under this Amendment Number 4 is $8,500 (Eight Thousand Five Hundred
Dollars and No Cents). This amount is to be paid from design funds budgeted for reimbursable design
requirements within the agreement with Donadio and Associates. This action has no effect on the Guaranteed
Maximum Prices (GMP) established for the construction of the facilities included under the contract (AIA
Document A121 CMc-2003 and AGC Document 565 . A Standard Form of Agreement between Owner and
Construction Manager where the Construction Manager is also the Constructor) and its Exhibit (AIA Document
A20M997 General Conditions to the Contract for Construction), as approved by the Board of County
Commissioners on April 19, 2005 , all as amended.
The total fee amount for The Project is then $2,370,300 (Two Million, Three Hundred Seventy Thousand
Three Hundred Dollars And No Cents

By the execution of this Amendment Number 4 as approved on the 16'h day of January 2007, this amendment is
hereby incorporated into the Standard Form of Agreement between the Owner and Architect, AIA Document
B141 - 1997, as modified, that was entered into by the parties on May 13 . 2003 .

For the following Project :
Indian River County New Administration Complex to be generally located on approximately 13 acres north
of 26" Street east of 20th Avenue south of Aviation Boulevard and west of Lateral "E" Canal in Vero Beach.
Florida. This Project shall also include but not be limited to other such improvements as the Extension of
20th Avenue to the North and construction of a Bridge across the Main Relief Canal connecting 20th Avenue
with Aviation Boulevard.
THIS CONTRACT SHALL HEREINAFTER BE KNOWN AS THE "PRIMARY CONTRACT"
OTHER AMENDMENTS
AMENDMENT NO. 1 :
On June 22 , 2004, the Board of County Commissioners approved Amendment No. 1 for the design of a third
building to be located on 43rd Avenue which will accommodate the Supervisor of Elections main office,
Buildings and Grounds and Animal Control facilities
AMENDMENT NO. 2 :
On Tuesday September 13 , 2005 , the Board of County Commissioners approved Amendment No. 2 for the
design of a fourth building, Emergency Operations Center (second facility to be located on 43`d Avenue) to
be located on 43`d Avenue which will accommodate the Emergency Services department.
AMENDMENT NO. 3 :
On July 11 , 2006, the Board of County Commissioners approved Amendment No. 3 for the design of certain
modifications to the Primary Contract including the deletion of the Emergency Operations Center function
from Building 2 and related improvements to convert the former EOC space into shell space for future
expansion. Also included was design of a 10,000 square foot second floor area on a portion of Building 2
which was formerly one story.
THE REQUESTED AMENDMENT ( AMENDMENT NO. 4) MODIFIES THE SCOPE OF WORK
AS FOLLOWS :
On Tuesday November 11 , 2005 , the Board of County Commissioners approved a request to proceed with
design and modification the meeting room in the former Supervisor of Elections space in Building 2 with the
understanding that staff will return for Board approval of an appropriate Change Order as needed. This
particular request is for an Amendment to the agreement with Donadio & Associates for the necessary design
to accomplish this work .
THE REQUESTED AMENDMENT ( AMENDMENT NO. 4) MODIFIES THE SCOPE OF WORK
AS FOLLOWS :
The Owner and Architect agree as follows :
To design a Conference/Meeting room in the former Supervisor of Elections space in Building 2 . The subject
space (approximately 1185 square feet in size) is to be reconfigured as necessary to provide a single
conference room area. The subject space had been partially completed when the decision was made to
proceed with this work . Limited improvements are required . Architectural and MEP engineering services
shall be provided as necessary to comply with applicable Regulation/Code requirements resulting in
approved construction documents suitable for construction.
Schedule
All preparation of Construction Documents is complete and has been transmitted to Turner Construction for
pricing.
Other Requirements
All elements or combination of elements of the design shall conform to Applicable Regulatory requirements
of Agencies having jurisdiction over the Project as a whole or in part.
