and the Architect:
Donadio & Associates, Architects of Vero Beach, Florida
For the Project:
Indian River County New Administration Complex to be generally located on approximately 13 acres north of
26th) Street. east of 20th Avenue. south of Aviation Boulevard and west of Lateral "E" Canal in Vero Beach.
Florida. This Project shall also include but not be limited to other such improvements as the Extension of 20th
Avenue to the North and Construction of a Bridge across the Main Relief Canal connecting 20th Avenue with
Aviation Boulevard.
AMENDMENT NUMBER 3 :
Authorization is Granted to proceed with payment of professional fees for Additional Services provided
previously as described in Exhibit A attached to this Amendment Number 3 .
As Follows :
• Deletion of EOC from Building #2 - Additional A/E Fee = $26,700.00 (for all related services)
• Design for a 10,000 Sq. Ft Addition to Building #2 - Additional A/E Fee = $30,500.00 (for all related
services)
Compensation:
The architect's compensation for all services under the Primary Contract including Amendments 1 & 2 is
currently $2,286,600. (Two Million Two Hundred Eighty Thousand, Six Hundred Dollars and no cents).
Total increase in professional fees under this Amendment Number 3 is $57,200 (Fifty Seven Thousand, Two
Hundred Dollars and no cents).
The total fee amount for The Project including Amendments 1 through 3 is then $2,343,800. (Two Million
Three Hundred Forty Three Thousand, Eight Hundred Dollars and no cents)
By its execution on this 11 ° day of July 2006, this Amendment Number 3 is hereby incorporated into the
Standard Form of Agreement Between the Owner and Architect, AIA Document B141 -1997, as modified, that
was originally entered into by the parties on May 13 . 2003 ,

2 . On Tuesday, February 8, 2005, the Board of County Commissioners approved a motion to move
forward with the design and construction of a second floor on a portion of Building 2 (approximately
10,000 square feet in size) which was formerly one story. Additional design work was required to
accomplish this such as architectural and structural modifications including the addition of a new
stairway , change from a roof structure to a floor structure etc . and increasing the height of the
building. This work also includes Civil Engineering site changes resulting in increased parking
requirements and new site approvals . There were also changes to the Mechanical, Electrical and
Plumbing plans in order to accommodate this expansion.
ADD A 2"p FLOOR TO CERTAIN PORTIONS OF THE BUILDING 2 PLANS, THE
ARCHITECT HAS NOT BEEN COMPENSATED FOR THIS REQUIRED ADDITIONAL
DESIGN WORK.
THE ARCHTECT ' S COMPENSATION FOR ALL SERVICES UNDER THE PRIMARY CONTRACT
INCLUDING AMENDMENTS 1 & 2 IS CURRENTLY $2,286,600.
COMPENSATION FOR ADDITIONAL SERVICES AS DESCRIBED ABOVE AND AS FURTHER
DESCRIBED HEREIN IS REQUESTED AS FOLLOWS :
• Deletion of EOC from Building #2 - A/E Fee = $26,700.00 (for all related services)
• Revisions to Building #2 floor plan Donadio & Associates $5,500.00
• Revisions to Building #2 door schedule, finish schedule, interior design drawings
and reflected ceiling plan. Song & Associates - $5,500.00.
• Revisions to Building #2 electrical voice and data plans, fire sprinkler plans ,
plumbing plans, HVAC floor plans, resizing of the electrical service and emergency
generator, resizing of the chiller. TLC Engineering - $12,800.00
• Revisions to Building #2 2nd floor structure. Jenkins & Charland - $1,200.00
• Building #2 EOC, DCA Structural Evaluation to determine feasibility and scope of
work.- Jenkins & Charland - $1,700.00
• 10,000 Sq. Ft. Addition to Building #2 — A/E Fee = $30,500.00 (for all related services)
• Revisions to Building #2 floor plan, elevations, roof plan , building sections, wall
sections and addition of a third exit stair. DAA - $7,500.00
• Revisions to Building #2 electrical and fire sprinkler floor plans, resizing of the
electrical service and resizing of the chiller. TLC - $9,500.00
• Revisions to Building #2 foundation plan, 2"d floor structure, roof structure, wall
sections, foundation and structure for the 3rd exit stairs . Jenkins - $8,000.00
• Revisions to the site plan for the additional building square footage, additional
parking and addition of the 3`d exit stair. Modification of the site plan and approval
with the City of Vero Beach. Masteller and Moler - $5,500,00
Total combined additional amount requested for the above services is $57,200.
This amount of fees is to be paid from project fund contingency funds budgeted for unforeseen additional
design requirements This action has no effect on the Guaranteed Maximum Prices (GMP) established for the
construction of the facilities included under the contract with Turner Construction Company, as approved by
the Board of County Commissioners on April 19 2005 as amended
The revised fee amount under the PRIMARY CONTRACT as a result of this Amendment 3 will then be
$2,343,800 (Two Million Three Hundred Forty Three Thousand, Eight Hundred Dollars and no cents).
All terms and conditions shall remain the same as in the "PRIMARY CONTRACT " except as noted
herein.

For the following Project:

PHIS CONTRACT SHALL HEREINAFTER BE KNOWN AS THE "PRIMARY CONTRACT"
OTHER AMENDMENTS
AMENDMENT NO. 1 :
On June 22, 2004, the Board of County Commissioners approved Amendment No. I for the design of a third
building to be located on 43rd Avenue which will accommodate the Supervisor of Elections main office,
Buildings and Grounds and Animal Control facilities .
AMENDMENT NO. 2:
On Tuesday, September 13 2005 , the Board of County Commissioners approved Amendment No.2 for the
design of a fourth building, Emergency Operations Center (second facility to be located on 43rd Ave.), to be
located on 43rd Avenue which will accommodate the Emergency Services Department,
REQUESTED AMENDMENT NO. 3:
The subject of this amendment to The Project (PRIMARY CONTRACT) is to make certain modifications to
the scope of design of the above Primary Contract in response to needs which have been approved by the
Board of County Commissioners as follows:
1 . August 2, 2005 , deletion of the Emergency Operations Center (EOC) from the plans for Building 2
of the Primary Contract. Also included in said deletion is design work necessary to configure the
former EOC area to a shell space for future expansion. This action was taken in conjunction with
authorization of a new contract for design of a new freestanding EOC on 43`d Avenue under
Amendment 2 as described above.
DELETION OF THE EOC FROM THE BUILDING 2 PLANS THE ARCHITECT HAS
NOT BEEN COMPENSATED FOR THIS REQUIRED ADDITIONAL DESIGN WORK.
