You are directed to make the following changes in the Contract Documents :
1 . Description : Reconcile the Unit Price Items in the Contract
Reason : The original Contract as bid , estimated the quantities for certain items of work . To
properly complete the work the quantities changed as allowed by the Contract. These items
were quantified by the consultant engineer as follows :
A . Item 2 . 04 lineal feet of curb increased from 110 ' to 270 ' at a bid price of $20 . 50 per
foot Net Increase $3 , 280 . 00
B , Item 2 . 03 square yards of sidewalk decreased from 586sy to 492sy at a bid price of
$29 . 69 per sy for a Net Decrease $21790 . 86
C . Item 2 . 02 square yards of asphalt paving increased from 2390sy to 2500sy at a bid
price of $ 19 . 39 per sy for a Net Increase $2 , 132 . 90
ADDITION to the Contract Price ( Items A, B , & C ) $ 2 , 622 . 04
2 . Description : Additional quality and quantity of plants adjacent to the new sanitary lift
station :
Reason : The size , type , and quantity of plants were increased from that originally
provided by the Contractor to more closely match the property owner' s type that
existed prior to the start of construction .
ADDITION to the Contract Price $450 . 00
3 . Description : Re-align 39 parking stall car stops :
Reason : As required by Public Works traffic division upon inspection of the original
installation by the Contractor the stops had to be installed normal to the stalls
resulting in removal and reinstallation : Increase $2 , 028 . 00
ADDITION to the Contract Price $ 29028 . 00
4 . Description : Contractor's Bond , Overhead , and Profit Costs :
Reason : Jo provide an amount for the contractor's overhead management and profit cost
at 10 % and bond premium cost of 2 % for items 2 and 3 above .
ADDITION to the Contract Price $ 302 . 32
5 . Description : Claim for damages from AT & T ( $835 . 70) and payment of the 1 / 15/09
through 2/ 12/09 water bill in the amount of $834 . 46 .

Reason : AT & T submitted evidence that Ocean Gate General Contractors Inc . damaged
their cable during the project construction . The Contractor is required by contract to pay all
utility costs during construction .
